The patient, Mr. [redacted], first came to my office for an emergency on 5/28/2014. He was sent to our office by a gentleman named Matt [redacted]. Apparently Mr. [redacted] was at [redacted] restaurant and bit into an avocado causing his tooth to break. He threatened to sue...

the restaurant due to this. Mr. [redacted], who owns the insurance company for this Denny's, sent Mr. [redacted] to me to repair the broken tooth #8. Mr. [redacted], the insurance guy, paid for Mr. [redacted]'s dental treatment which totaled $2,325.00. The dental treatment was a root canal therapy, a build u p and crown. Mr. [redacted] did NOT  pay for any of this treatment. Mr. [redacted] paid for Mr. [redacted]'s treatment. Mr. [redacted] was called twice by my office, 5/29/2014 and 6/02/2014, to see how he was doing. At both conversations Mr. [redacted] said he was doing well. On 7/10/2014, Mr. [redacted] came back to my office for a deep cleaning, comprehensive exam and filling on tooth #11 F. At this time I reviewed with Mr. [redacted] other teeth that needed dental work due to severe decay and periodontal disease. This treatment cost a total of $1028.00 which Mr. [redacted] paid for $1000.00 with his MasterCard and the remaining $28.00 was written off as cash discount courtesy.  The next day, 7/11/2014, a care call was done to Mr. [redacted] and once again he said he was doing well. On7/30/2014, Mr. [redacted] came in to the office because he broke tooth #8 (the tooth I repaired from the previous trauma) again. I placed a post in this tooth and retrofitted the crown for him at no charge. Mr. [redacted] paid nothing for this dental work. I did tell him I was not sure how long this tooth would last because this time he broke the tooth at the gumline. The next day, 7/31/2014, a care call was done and Mr. [redacted] said he was doing fine and the crown was in and the crown was good. That was last time Mr. [redacted] and my office had any contact until yesterday, 8/19/2015, 13 months later. Mr. [redacted] came in to my office yesterday saying the crown was loose. We asked him if he had any dental insurance. He said he did have dental insurance. My office manager asked to see his card so insurance can be verified and any dental services rendered would be filed with his insurance. Mr. [redacted] did not have a dental insurance card on him. So my office manager told him that for today's visit what the price of an emergency exam and one x-ray would be but she was going to look under his employer, [redacted] restaurant, to see what dental insurance the company carried and get Mr. [redacted]'s information that way and file with his insurance company. When my assistant went to take an x-ray of Mr. [redacted]'s tooth, Mr. [redacted] got up out of the chair and started complaining that he was not going to stand for this and he was not going to pay for anything and he walked out of the office.     Now to address Mr. [redacted]'s complaint. 1) The original dental treatment done on Mr. [redacted] was NOT paid by Mr. [redacted], 2) Mr. [redacted] claims when he came back to my office for the crown being loose I was going to charge him again for the work. This is completely untrue. Mr. [redacted] came back to my office regarding this tooth when he broke it again which I took care of at NO CHARGE. 3) He claims he was charged $1200 for a teeth cleaning and filling which is not true either. He was charged $1028 for a comprehensive dental exam, full series of xrays, 4 quadrants of deep scaling and root planing and one filling. Mr. [redacted] paid $1000 of the bill and I wrote off $28.00. 4) Mr. [redacted] claims my office kept a copy of his medical savings card and charged $1200 to this card without permission. This is untrue. My office does not keep credit card or medical savings credit card numbers on file. My office did not charge $1200 to his card. My office charged $1000 to his card and with his permission. And lastly 5) Yesterday. Mr. [redacted] did not give me a chance to complete my exam on his broken and did not give me a chance to discuss treatment recommendations and what I would do for him. He simply got up out of the dental chair and just walked out of the office.
