The veterinarian, Dr***, admitted he was not accredited by the USDA APHIS to sign off on my cat's USDA APHIS forms required for export of cat to France He offered no information or consultation of steps to follow to contact another veterinarian, or the USDA directly I had to find another veterinarian who was and proved they were accredited by the USDA APHIS who issued the correct documents, filled in completely and correctly, and with step by step instructions on the procedure to follow I followed their procedure to conclusion, successfully Dr***'s office had no information to obtain the correct forms or how to fill them out, or where to send, or whom to contact He is not accredited by the USDA APHIS Check the website and verify for yourself that he or Rancho San Carlos Pet clinic is not listed The matter is closed I found services with another veterinarian I had to pay additional fees.]

correct. The certificate reference number is simply the normal Accredidation number which was clearly printed on the official health certificate needed to travel, and provided to Ms. [redacted]. Ms. [redacted] stated that she spent $942.43 for this preparation, which is not accurate. This figure included a previous medical procedure for her pet. The total charged for the health certificate was $80.46.
