RE: ID# [redacted] Dear Revdex.com Representative;We are writing in response to the 10/31/consumer/customer complaint, ID# [redacted] .The following consumer statements are accurate:*Heritage Valley Sewickley accounts paid on April 18, 2016; two separate transactions.*First transaction at 2:pm in the amount of $posted to account [redacted] Second transaction at 2:pm in the amount of $also posted to account [redacted] When the customer service representative erroneously posted both payments to the wife's account ( [redacted] ), it caused a credit balanceThe Credit review representative identified the patient/wife to have another open account ( [redacted] ), for $10.14...*HVS representative moved $from account [redacted] to account [redacted] to satisfy the open balance.*The difference between $and $is $42.04, which is amount Heritage Valley Sewickley refunded to the patient (wife).Since each individual is assigned their own account number for each hospital visit the Credit review representative did not realize the credit balance on account [redacted] was meant for the husbands account number [redacted] I see there are notes in the system on 11/1/that all of the above was explained to him and on 11/3/called back to make a payment of $52.18.Please know that Heritage Valley Health System is committed to being a leader among community health systems nationally by providing exceptional health services through clinical proficiency and excellent serviceWe thank you and our consumers for expressing and informing us of their concerns.Sincerely,Christine T [redacted] , Manager Patient AccountsHeritage Valley SewickleyHeritage Valley BeaverPh#: [redacted] ***Email: [redacted]

[redacted] paid $154.00 with a provider adjustment of $69.58 leaving a member responsibility of $84.42. [redacted] The consumer did fax a copy of his HSA spending documentation on 8/10/16 showing he submitted $308.00 to Heritage Valley Sewickley.Please be advised that I have researched our system to the best of my ability with the supplied documentation. The problem is the documentation supplied by the consumer was not sufficient to trace the payment of $308.00 presumably sent to our office. To trace the $308.00 payment, a copy of the credit card confirmation is required order to determine what banking institution received the $308.00 payment that the consumer paid through his HSA account on 5/9/16.Please know that Heritage Valley Health System is committed to being a leader among community health systems nationally by providing exceptional health services through clinical proficiency and excellent service.
