Herman Area District Hospital

The first notice was not received by the Hospital.  The second notice was not received from Revdex.com until March 16 due to the incorrect address for the Hospital.  The hospital po box is 470 not 19.  I have reviewed comments on each of the 3 accounts beginning on 4/27/14 through...

9/29/15.  On 8/13/15 a Financial Assistance form was mailed to her.  The application instructions mailed with the form state the application should be returned with information within 7 days of date given. We received the form back in November 2015 and it was signed on 10/20/15, over 2 months late.  Regular collection process is followed until the application and required information is received.  Her application was received after we sent her earlier accounts to a collection agency.  Our Assistance policy is not part of any larger governmental plan.  The instructions also state the assistance does not apply to accounts in bad debt.  Since the application was received after the accounts were written off to the collection agency, we cannot apply charity against those accounts.  Sincerely, Business Office Director
