On 3/31/patient in question came in anxious and complaining of severe excruciating pain to her upper anterior teethUpon oral examination patient was partially edentulous on upper with carious isolated teeth #and #9; one periapical X-Ray was takenPeriapical widening on the apices of root #and #there was no swellingOne carpule of 2% Lidocaine with Epinephrine 1:100,and infiltration technique was usedAfter waiting for a few minutes patient was asked if she was numb, but she just continued to complain about her pain and wanting to have teeth taken outTeeth was luxated and extracted then packed with x gauzePose extraction instructions and RX: Norco 5/325mg, Disp: tabs, Sig: tab Q 4-hrsPRN pain givenNothing extraordinary with this treatment; was a simple extraction
After treatment patient continued to complain but this time about her treatment
On Wednesday 04/02/14, I received a miss call from patientThere is no clinic on Wednesday and I had no signal at my locationWhen I returned the call no one answeredSo 04/03/I asked my receptionist to call the patient to see what was neededPatient said she wanted pain medication and that she was in a lot of painRX was called into Walgreens ###-###-#### for Amoxicilin mg, Desp: tabs, Sig: tab t.iduntil gone and Vicodin 5-mg, Disp: tabs, Sigtab Q 4-hrsPRN pain
Patient then called on 04/10/wanting more pain medicationA staff member advised her to come in so that we could evaluate the extraction siteShe adamantly refused saying, 'she doesn't want to be seen by that man ever again' with a few choice wordsShe was advised that medication was prescribed twice before and that she should be seen by the doctor to make sure all was wellPatient advised that she would see another dentist.
On 04/28/patient called with the same complaints about the doctorWhen she was advised that she would be seen by an associate doctor an appointment was madeAgain, she came in anxious and extremely verbalPatient reported to the associate doctor that her whole mouth hurt since she had the extraction and was extremely unhappy with her experience her experience hereShe wanted more medication for the painUpon evaluation another periapical X-Ray was takenIt showed that there was healing of bone and tissue on extraction site.Patient was given a referral to see an Oral Surgeon for possible further complications outside of general realmAssociate doctor did not give pan medication
