07/06/2015To Whom It May Concern:I am responding to this letter from the Revdex.com, (complaint ID # [redacted]) for patient [redacted] stated in his complaint that he was not the guardian of [redacted] We have [redacted] as the Head of Household on this account. At the time (06/04/2013)...

[redacted] was under [redacted]'s dental insurance with [redacted]. There were services rendered to [redacted] on 06/04/25013. The grand total for the crown and RCT was $1731.00 for date of service 06/04/2013. Insurance had paid a total of $842.20, in which left the patient a balance of $888.80. Patient made a payment of $709.70 on 06/04/2013, the amount still owed was $179.10. [redacted] also had a previous balance of $91.60 from 2013. The patient is responsible for any unpaid charges due to exclusions and limitations. It is our legal right to collect on these services.[redacted] also had a $50.00 balance on his account for date of service 04/25/2013, his insurance charged a $50.00 deductible for the two restorations.We do send monthly statements to our patients for unpaid balances, unless there is a pending claim, the patient will not receive a bill from Hillrise Dental. I feel we did not harass this patient by sending him a bill from our office.I have it documented that I tried to contact [redacted] on 07/03/2015, in which I had left a message. I also tried calling [redacted] on 07/06/2015, I could not get thru to her voicemail. I also contacted the Revdex.com and spoke with [redacted]; she told me that I needed to respond to this complaint in writing.I am willing to write off [redacted]'s 2013 balance of $91.60, which leaves [redacted] a balance of $50.00 and [redacted] a balance of $179.10 fora total of $229.10.If you have any questions please contact [redacted]
