Our office provided an exam, x-rays and a periodontal maintenance cleaning for [redacted] on 12/21/ We have seen [redacted] for periodontal maintenance cleanings from 2009-and there has always been a co-pay with his cleanings as per the insurance he had during this time [redacted] provided us with his new insurance information at that time and we called to verify benefits and were able to send his claim to them His insurance applied his $Deductible towards his periodontal maintenance cleaning We tried on several occasions to contact the patient in regards to this balance but according to the personal information [redacted] has provided with this filing we had a different address, phone # and e-mail on file This has now all been updated in our systems Our office has contacted our collections agency and asked to have his account closed with them so we may be able to resolve this in house and they have made the exception to do so [redacted] now has a balance with our office of $and we would be happy to take care of this balance in house Please have him call our office @ ###-###-#### Thank you

provided us with his new insurance information at that time and we called to verify benefits and were able to send his claim to them.  His insurance applied his $100 Deductible towards his periodontal maintenance cleaning.  We tried on several occasions to contact the patient in regards to this balance but according to the personal information [redacted] has provided with this filing we had a different address, phone # and e-mail on file.  This has now all been updated in our systems.  Our office has contacted our collections agency and asked to have his account closed with them so we may be able to resolve this in house and they have made the exception to do so.   [redacted] now has a balance with our office of $103.35 and we would be happy to take care of this balance in house.  Please have him call our office @ ###-###-####.  Thank you.
