March 1, [redacted] ** *** RE: Aspen Dental Account Number [redacted] Revdex.com ID # [redacted] Dear [redacted] : Thank you for sharing your concerns about the treatment you received I am writing to you on behalf of the Cincinnati, OH Aspen Dental branded practice, owned and operated by McQueen Dental Investments Inc I certainly do apologize for your experience at Cincinnati, OH Aspen Dental officeI am in receipt of your complaint filed with the Revdex.com I have reviewed it and your Aspen Dental account Upon reviewing the [redacted] claim payment issue with my insurance department, I have found that [redacted] did “not” remit payment for the services you received on May 02, The electronic response and supporting documentation that we have on file from [redacted] shows that the $estimated insurance payment for the cleaning, exam and x-rays treatment that you received was not paid The Human Readable Remittance Notice submitted by [redacted] show the Check/EFT # as [redacted] and the NPI # as [redacted] (page 1) It shows a zero amount paid As a prior courtesy, Aspen Dental removed part of the balance due for the May 02, date on December 04, As a final courtesy we have removed the $remaining due If additional documentation is available that supports your assertion that payment was made by ***, please forward it to my attention and I will re-visit this Even though your Aspen Dental account now reflects a zero balance, I encourage you to contact [redacted] and request they remit the missing $payment I appreciate you giving me the opportunity to respond to your inquiry A copy of this letter has been forwarded to the Revdex.com in response to your complaintI am available for any further questions at the below contact informationSincerely, Ron H*** Ron H***Senior Patient Satisfaction SpecialistAspen Dental Management, Inc.Phone: 1-315-454-6000, Extension ***Fax: (315) 295-

[redacted] RE: Aspen Dental Account Number [redacted]        Revdex.com ID # [redacted] Dear [redacted]: Thank you for sharing your concerns about the treatment you received.  I am writing to you on behalf of the Cincinnati, OH Aspen Dental branded practice, owned and operated by McQueen Dental Investments Inc.  I certainly do apologize for your experience at Cincinnati, OH Aspen Dental office. I am in receipt of your complaint filed with the Revdex.com.  I have reviewed it and your Aspen Dental account.  Upon reviewing the [redacted] claim payment issue with my insurance department, I have found that [redacted] did “not” remit payment for the services you received on May 02, 2017.  The electronic response and supporting documentation that we have on file from [redacted] shows that the $59.00 estimated insurance payment for the cleaning, exam and x-rays treatment that you received was not paid.  The Human Readable Remittance Notice submitted by [redacted] show the Check/EFT # as [redacted] and the NPI # as [redacted] (page 1).  It shows a zero amount paid.  As a prior courtesy, Aspen Dental removed part of the balance due for the May 02, 2017 date on December 04, 2017.  As a final courtesy we have removed the $17.00 remaining due.  If additional documentation is available that supports your assertion that payment was made by [redacted], please forward it to my attention and I will re-visit this.  Even though your Aspen Dental account now reflects a zero balance, I encourage you to contact [redacted] and request they remit the missing $59.00 payment.  I appreciate you giving me the opportunity to respond to your inquiry.  A copy of this letter has been forwarded to the Revdex.com in response to your complaint.
