Hello my name is *** I am writing regarding my grandmother ***.
Her caregiver company is "Homeliving Health Provider" located in ***
She has been released from the hospital since January 17, 2020 and since then she had her caregiver company" Homeliving Health Provider" sending their caregivers until end of the January 2020. Since then, I don't know what's the reason they stopped sending their caregivers to her. She has been calling them, asking why they stopped, they don't give any reasonable answer, they say our caregivers now working for their license , they can't come now. *** is really in extreme condition, she is stuck in bed, she has wound on her legs , on her body, she can't move, she really needs for a caregiver. Please take into consideration for her situation. She really needs the help.
Other (requires explanation) I just need "Homeliving Health Provider" send their caregivers to my Grandmother. and I want it to be done regularly, please!
Waiver Personal Care Services (WPCS) concluded on 2/29/2020. Beneficiary was transferred to the county appointed agency. Provided discharge instructions to the daughter/family.
