1.) Issue: The last name of both infants were spelled incorrectly on their birth certificates and social security cards.

Complication:

Babies lost health insurance because we could not show proof of their

existence with legal documentation and could not vaccinate them or get

them to a pediatrician without major bills.

2.) Issue: Wrong Insurance billed with a wrong last name for myself. Billed as [redacted]

Klusovsky to my husband's insurance UHC Select... My name is [redacted], my insurance is UHC UMR.

Complication: Health insurance

denied all claims. I have been talking with insurance companies every

day for multiple hours a day for 10 months.

3.) Issue: The babies were automatically put on my husbands insurance in error.

Complication: His insurance will not remove the babies from his insurance because

this would be "removing care from a minor" and now follows the "birthdate rule". My insurance is denying all

claims for the babies birth because they say, "the babies have alternate

health care coverage" and I am left with TWO deductibles for TWO family insurances.

These three issues have caused me so much grief. Over 300

phone calls, hours and

hours of my time. I am in constant contact with your billing department who is trying to get this

under control. After 10 months I am still receiving HUGE bills from labs,

anesthesiology, first assists, ultrasounds and numerous people who read

the tests when I have already paid 3K and met my

deductible.
