Hood to Coast Healthcare is attempting to double bill me for labs done in December 2019. They sent my labs to Providence and Providence billed me, which I paid in January 2019, then Hood to Coast sent me a bill in March for the exact same line items that were listed on the Providence bill. I notified them in writing in March that I had already paid for these through Providence, and in April received another bill. I contacted the office manager via email (who had told me to contact her for any billing/claims/etc.) and sent a scan of the bill, and she tried to claim it was for something different even though it was not, then tried to tell me to "contact billing" even though they are the ones who billed me! This is EXTRAORDINARILY UNETHICAL and fraudulent billing practices. I told them in writing twice this is a double bill and will not be paying it, and that I would be filing a complaint against them to the Revdex.com.

Note: If you look up old reviews of this business's other locations both with their current name and the prior names used, there are multiple complaints about unethical business practices, including billing.

In addition, they have incorrectly coded claims to my insurance multiple times and never sent updated statements as promised, have misled on their website and when scheduling appointments (claiming to offer Skype appointments when they did not even have Skype set up - or any telehealth at the time I first scheduled!), doing a telephone appointment with me for the second then later, claiming they only do them by the Zoom (a video service with a terrible cybersecurity record) which was a lie!) as well as emailing patients trying to sell "coronavirus treatment" packages in their newsletter which they had to go back on in a later email because this was extremely unethical.

completed 2 sets of labs. One set of labs went through Providence. This was for blood work completed. See attachment for Providence labs. She also did a second set of labs through Doctor's data/Labrix which was a saliva test for hormones and adrenal functioning. These are actually 2 different sets of labs, with 2 completely different companies with 2 different CPT billing codes.

We have sent *** all copies of the labs.

I have included them here to help with this manner. She is not being doubled billed, she is only being billed for all the labs that she had completed.

We have instructed patient to call the billing department to clarify.

I am rejecting this response because: once again, the bill that you sent me (attached), is for the exact same line items that is on the Providence bill. I did not ever request to get tested (nor billed) twice for the exact same things, and the bill you are sending me does not reference any saliva samples, it specifically mentions blood samples including venipuncture, which I was told were sent to Providence. I already contacted billing in writing at the address for Hood to Coast in March and had provided documentation of the double billing.
Attached is proof.

Hello,

2 tests were completed. One was a blood draw completed on 12/21/19. The other was a saliva test completed on 12/22/19.

Patient has both copies on each tests. The blood draw was through Providence. The saliva test was completed at home by the patient and through a company called Labrix/Doctors Data. The patient completed both tests. The blood draw showed some basic screening labs as well as thyroid and some hormones. The saliva test was an analysis of her free and active hormones as well as her cortisol levels which addresses her adrenal functioning.

Yes, some hormones were completed in the serum/blood as well as through the saliva, but these are completely different labs that analyze completely different things. Hormones in blood/serum show the total circulating hormones. These hormones are bound to proteins called SHBG - these show what the body is producing but not necessarily what the body is using. The saliva hormones are the free and active hormones in the body - these are the hormones that the body is actively using. Since the patient had a very complicated hormone history, I felt it was necessary to get a complete picture of her hormone functioning (it terms of what she is making and using) in order to help the patient in the most accurate matter.

I am sure that the patient remembers completing both tests. The patient also greed to doing both tests.

I am rejecting this response because I have already provided proof of payment multiple times made to Providence. Dr D *** said in our appointment at her office that my labs would go through Providence and I paid Providence. Her statement that she decided to test the same hormones twice without does not negate the fact that I've paid the bills for hormone testing through Providence already. I have a serious problem with the ethics of a non-medical doctor who has a history of changing her story, everything from claiming she had Skype for appointments when it wasn't actually set up, doing appointments over the phone then later trying to force patients to use non-HIPAA compliant video software and claiming she didn't do telephone appointments (even though she had), to refusing to answer post-appointment treatment/follow-up questions (instead trying to force patients to make another appointment), to sending out marketing emails during the pandemic about ways to "treat" coronavirus. This office has a long history of reviews about their sketchy business practices, from billing to care, and I will not be manipulated by Dr D *** for even more money than I've already paid to them, Providence, and CMH.

During my initial consultation at Hood to Coast Healthcare, I told Dr. Samantha that I wanted to lose weight, tone up and most importantly feel good. Since I started training, I've lost 20lbs, almost 9% body fat, and 25 inches and I feel amazing. My energy level has increased, and I have a new confidence, where I now enjoy clothes shopping and socializing more. I cannot imagine a time where I no longer attend these sessions as they have changed my attitude towards food and exercise and how I feel about myself. Thank you so much I would highly recommend!

I went to the Hood to Coast Healthcare intending to change my eating
habits and if weight loss occurred then it would be a happy side effect. The

In spring 2017 (I don't have the exact date) I came in for an appt to review a lab test to see how the supplements and prescriptions I had been taking were affecting my medical condition. I informed Dr. S that I continued to take the substances she prescribed to me when I gave the sample for the test. She told me that by doing that I negated the results on the lab. I informed her that her office staff instructed me to continue taking my substances during that sample period. Later when I got my bill I called her billing department,and finally the Hood River office staff to remind them about the botched test. I was told I'd get a call back after the office person spoke to the Dr. We went back and forth for months. Sometimes because Dr.S's staff didn’t follow up with my calls and sometimes when I didn't call back immediately. (I have a summer job where I can’t make phone calls mid week during business hours and I also travel outside of the country for weeks on end) I scheduled an apt. to get retested before a 5 week trip.When I showed up for that apt Dr. S cancelled because her dog died. I understand things like this happen. Eager to get my appt done before I left I agreed to new appt date but didn’t get it into my calendar. (My fault) I missed the appointment, Dr. S charged me a $100 missed appt fee.Eventually, I started to get late fees for the lab and missed appt fee.I contacted office multiple times this fall trying to resolve this issue.Eventually, the office told me to sign up for an online system that would allow me to talk to the Dr.I did that, explained myself, asked for clarification and received no response. I wrote three messages trying to explain myself and ask for resolution including offering to pay for the appointment where I came in to review the botched lab. After my third email I received an email from her attorney saying all fees stand.I have more details but no room here to include them. Thank you for your help.

Dear Ms.:

I’m the attorney for Hood to Coast Healthcare. Given past issues with communications and non-responsiveness from you, Dr. Du M asked me to step in and clarify the pertinent issues in an effort to finally resolve the fees you owe to the clinic for services. I’ve pasted what I believe is your last correspondence on the matter below.

You claim the instructions were unclear, though you do not know how since you did not retain them. You can find a copy of what you received attached to this email. The applicable section for the problems with your results is captured in screenshot below.

It is my understanding you called the clinic prior to collecting samples and asked if you could continue to take supplements. You were told, “yes,” as supplements do not conflict with the test. However, it is my further understanding you are on a topical hormone, which is what interfered with the test. The above instructions you were provided clearly state that you should cease use of a topical hormone throughout the entire day of collection – an instruction in all caps, prominently included in the IMPORTANT Guidelines section.

Your failure to either read or understand this straightforward instruction is your fault alone. Your inquiry to the clinic regarding supplements is irrelevant to the matter. Therefore, you will remain responsible for paying for the lab fees.

Furthermore, you claim there is something unfair about being charged a fee for failing to provide proper notice of an appointment cancelation, despite the clinic’s clearly stated policy that cancelations must be made more than 24 hours prior to the scheduled time. Given the clear policy regarding cancelations, as well as the general knowledge that canceled medical appointments will usually incur a fee if not accomplished in a timely manner, your argument on this point is also without merit. You will also find attached the original financial responsibility form signed by you, clearly indicating the appointment cancelation policy; and you were provided the updated policy on two separate occasions, noting the increase to a fee of $100.

You will be held to your responsibilities on both these matters. However, if you make timely payment of $320 for the lab fee and missed appointment, Hood to Coast will waive the $40 in late fees that have accrued. You indicate below you are traveling out of the country this Sunday. In order to take advantage of the offer to waive late fees, payment of $320 must be received prior to you leaving on your trip. Failure to make payment in full by then will result in the re-imposition of the late fees and the accrual of additional fees, including the $100 collection fee.

Please refrain from contacting the clinic directly on this matter; you may send any questions to me.

I am rejecting this response because:
I understand the financial responsibility that I have to cancel appointments 24 hours in advance. What I feel is unfair is that the only reason why I had to make the appointment that I then later missed is because Sam cancelled an appointment on me without 24 hour notice which I showed up for. I’m just asking that in all fairness that we waive that $100 fee since we both are at fault. Sam has not yet acknowledged that she cancelled an appointment on me and I’m frustrated in her unwillingness to see this from both sides and show flexibility on this policy.

About the supplements and medications. The instructions do not mention supplements which is what I called the office about. Furthermore, the instructions specifically say to call your medical provider if you are taking a cortisol supplement (which I am) for recommendation on whether to take the supplement or not. When I called the office I was told to take the supplements. When I went to the appointment with S she told me that I shouldn’t have taken the supplements and I told her that I called the office for clarification and was told to continue with the supplements. She said at that time that her office staff should have consulted her before giving me instructions.

Furthermore, if the supplements I’m taking to alter my hormones (I’m taking other supplements to alter my estrogen and DHEA) don’t alter the test then my question to Dr S was and still is; Are they effective? Why am I taking them? I’ve paid hundreds of dollars for supplements through her office that she prescribed to me that are supposed to be treating conditions that the hormone test in question was being used to determine their efficacy and progress.

I went to the appointment to discuss the test which in my mind was a waste of time since the test we were discussing wasn’t accurate. In an act of good faith, I’m willing to still pay for that appointment even though the reason for the appointment wasn’t addressed adequately. It is not fair to ask me to also pay for the test that was inaccurate due to information given to me from Sam’s office.

Sam's office has regularly been slow to respond to my phone calls or hasn't returned calls after saying they would get more information from S Often I had to call back to remind them I was waiting on information. In the past month I had to wait 10 days for a response from her office. I don't think it's fair for me to bear the responsibility of the late fees when Sam's office played a part in the unresolved nature of this situation.

I am rejecting this response because: It's the same response sent earlier without addressing my earlier response points. Here are the important parts that I'd like addressed:1. I called the office of Hood to Coast about the test instructions because they specifically say to call the medical provider if one is taking the type of supplement I was taking.2. At that time the Hood to Coast staff told me to take all medications while doing the test, contradicting the instructions that came with the test.3. When I came in to review the results, Dr. S said that office person should not have told me to take medications and supplements and that she'd have to "talk to her staff" about it.4. Despite getting incorrect instructions from Hood To Coast which negated the test, I was still issued a $200 bill for the test.It is not fair to ask me to pay the $200 for the test that Hood To Coast gave me incorrect instructions for and that was inaccurate due to those instructions. The business must take responsibility for the mistake of their staff.I want this issue closed and to remedy this I will pay for the appointment to discuss the faulty results. I will also pay for the $100 missed appointment fee even though there is no recourse for me as a patient to charge the business for cancelling the appointment on me. I will not pay for late fees. Let's close this chapter and move on. Will you accept my proposal?
