Complaint: [redacted] I am rejecting this response because: The business did not as they claim perform their "standard protocol" of a school visit for vision analysis, as we paid for an assessment in their office and do not desire a "school report"Their word usage of "Report" is attempting to circumvent the standard practice when conducting an assessment to fill out the assessment protocols, what we desire and feel we paid forOur child's school is not the interested party in this assessment, but the provider we are seeking to begin vision therapy services for our daughterWhat we were provided with for the $fee was very similar to what we received from her regular eye exam, and in no way was a Functional Vision Analysis as all the aspects of the forms that could be evaluated as such were knowingly left blank Sincerely, [redacted] ***

As indicated in my previous response, a school report and school visit are part of the vision therapy services, not a part of the Functional Vision Evaluation services I do not know what the family means by "Assessment Protocols," and what type of Report comes from them Please define and elaborate Another Developmental/Behavioral/Functional Optometric Physician would want a copy of my actual data in order to render a professional opinion and treatment recommendations Any type of report is intended for someone other than a like professional A Primary Care Vision Evaluation typically measures eye refraction and eye health Binocular (two eyes) findings are not usually a part of a Primary Care Vision Exam Binocular measures are a part of my Functional Vision Evaluation findings mailed to the family.The offer in my previous reply continues All of us wish the family the very best in making decisions for their child

Thank you for this opportunity to respond. Along with the family, we want the best for their daughter.The family brings their daughter for a Functional Vision Evaluation. Vision Therapy intervention is the treatment of choice for their daughter's vision issues. For all our... school-age vision therapy patients, standard protocol is to schedule a school visit to explain possible vision impacts on reading and learning performance and a school Report is completed. These services are a part of our vision therapy treatment program, and the fees for these services are included in the treatment fee. The family has chosen to receive vision therapy treatment with another provider. We totally support the family's decision - comfort and confidence with a provider is paramount.The treating provider is in the best position to provide a school Report, and other possible interaction with the school. If a patient not in vision therapy treatment wishes a Report, our standard fee is $150.00. The Report is often five to seven pages in length, and takes 45 to 60 minutes to complete.So the new provider has all of our findings, we have mailed copies of the child's records to the family. We are happy to provide any desired additional information and support to the treating provider.If the family still desires a Report, and financing the Report presents hardship, we are willing to negotiate a reduced fee. Have the family contact us.We strive to provide the very best for the families we serve.
