I never had a doctors appointment on 7/29/I have the the receipt for my very last doctors appointment and it was not on 7/29/The office is showing no record of the appointment I had which was on a later date than 7/29/I can provide the receipt of my last visit with the date and it paid in full with cash.

Revdex.com spoke to Stephanie at the Dr.'s office and Tammy at the billing office and the following was relayed and clarified: Patient was seen on March 13, and paid for that visit in May of 2013, $with check #***Patient was seen on 4/12/and paid for that visit in August of 2013, $with check #***Patient was seen on 2/7/but was not charged as this was a No Fault visit due to an accident and covered by auto insurancePatient was seen on 9/16/and stated that he had insuranceHe paid $for co-pay chargeIn fact, he did not have insurance and the remaining $was due, $of which was written off to assist the patientPatient paid $due on 12/19/with check # ***Patient was seen on 4/2/and was billed $of which $was written off for a total due of $to assist the patientThis was not paidPatient was seen on 7/29/and was informed that if payment was received within days that they would reduce the charge from $to $Patient returned on August 6th, and paid $cash which was applied to the 4/2/visit that patient had not paid forThe patient currently owes $for the July visit which has not been paidSee attached document for 7/29/appointment scheduled by patient on 7/28/
