Review: I asked them to fill out a medical sheet for my job and they said they would. Instead it was left on the doctors desk unattended too for over 3 weeks. I was also not informed that it was not filled out until my job told me. When I called asking that it be filled out I was told the doctor that could fill it out was out of the office and they "didn't know" when she would be back. When I asked if another doctor could sign it they said they would try but all the other doctors were "too busy" to sign my paperwork. I still have no sheet for my job and my insurance only covers one psychical a year so there is no other way for me to have this sheet filled out other than threw them.Desired Settlement: I would like my form filled out and completed by [redacted] 2/** and faxed to my job.
