I initially saw the person filing this complaint with his wife on 5/7/The wife was the only one that provided insurance
information, so the visit was billed under her insuranceOn 5/15/I saw the wife individually, so this was again billed under her insuranceI was scheduled to see the complainant that same day, but later in the afternoonUnfortunately I had to reschedule our meeting due to the school calling me to inform me that my son was illI made three attempts to call this individual to make sure that he knew I needed to reschedule our appointment; he did not answer or respond to the voicemail that I left(This appointment was not billed to his insurance.) The weeks following 5/15/15, my office staff person, as well as myself, diligently made attempts to get in contact with this person so as to get him rescheduled in a timely mannerWe did not hear back from him for the two weeks that followedI saw his wife individually again on 5/28/15; I told her that I was having a difficult time getting a hold of her husband and have left multiple messages but have not heard back from himI explained that I knew he needed my after 5pm appointment time slots so it did not inconvenience him from his workI continued to explain to her that these are the most popular times for many people and that these times are taken the quickest, so the sooner he could contact me, I would make my best attempt to leave some of those times open for him...especially because I had to cancel the last appointment we had scheduled due to my son being illDays after this conversation with his wife, my secretary was able to get a hold of him and schedule him for 6/10/15; she reminded him to bring in his insurance informationOn 6/10/15, the complaintant came to the appointment and brought his TotalCare EAP member card; I have no other insurance information regarding this individualI was not familiar with this particular EAP program, but explained that I would have my staff person check on itMy staff person did so and found out that I was not a provider for this EAP but could fill out an application in order to become oneOn 7/1/I received the application packetBecause it had been a while since I had filled our an application form, I needed to update my resume and gather numerous documents to fulfill all of the requirementsI made my best attempt to get the information faxed to ESI before I left on my week long summer family vacation the week of July 13th; Unfortunately, the ESI representative had given us the wrong fax number so we did not have success sending it through before we were all out of the officeUpon my return to the office on 7/20/15, we were able to get the correct fax number and sent it immmediatelyESI then sent the contract for me to sign; I returned it to them on 7/21/The ESI representative also explained to us that appointments that had occurred previous to this authorization may not be coveredThis is the reason that I have the following statement on my registration and contract form that all clients must sign: "I the undersigned, certify that I (or my depndent) have insurance coverage as noted above and assign directly to the healthcare provider listed at the top of this form all insurance benefits, if any, otherwise payable to me for services renderedI understand that I am financially responsible for all charges whether or not paid by insuranceI hereby authorize the healthcare provider to release all information necessary to secure the payment of the benefits and to mail patient statementsI authorize the use of this signature on all insurance submissionsThe complaintant signed this form on 6/10/when he brought in his EAP cardI can provide a copy if requestedIt should also be noted here that typically I have a hour cancelation notice, otherwise there is a $late cancelation feeOn 6/17/15, the couple was scheduled for an evening appointmentThey came minutes late as they had literally bought their first house that day and were busy movingI honored their request to reschedule with no late notice feeThe following week I did not have any evening appointment open, however, I was willing to stay later on Tuesday evening (6/23/15) from 5pm to 6pmThe couple canceled this appointment late as well, however, I did not charge them a late feeThus, I am confused at the complaint put forward as I believe I was more than accomodating
