*** *** ** *** * *** *** *** ** *** ** *** ** ***What I faxed you was the invoice I received from Immco on 4/8/with $1,amount duewhich states "Per BCBS this claim was previously processedWe did not receive any claim information as we are out of networkplease forward the EOB from BCBS"I also faxed you the EOB I received from BCBS 0n 1/22/which states "The plan does not include coverage for claims that were not received within the specified time limit for filing a claim."BCBS did not pay Immco because Immco did not submit the claim until months after the date of service which was beyond the specified time limit for filing a claim with BCBS. I am requesting again that Immco mail me an invoice stating there is zero balance due on my account.Thank you, *** *** *** ***

We received your letterPlease see below the explanation of what has happened with this accountThis patient has BCBS MN, we are out of network with this companyThey do not always sent us the explanation of benefitsWe submitted this claim to them on different occasions 10/26/2014, 12/17/2015, 1/13/and 2/9/On 12/17/and 2/9/the claim was submitted with proof of timely filingOn 2/22/we received an EOB from BCBS MN, the first one we receivedThey denied as a duplicate claimSo they did process it before and did not send us the informationI sent an invoice to the patient with a note stating that and requested they forward us the initial EOBWe have never received any type of payment for this claim . We have spoken to Mr*** a few times regarding this claim. The last attempt at communication was a voice mail on 7/15/to discuss a phone call that I had with *** regarding this claim on the same day. I left my name and number in the message and have not received a
response from that patient since that date. When I spoke to *** on 7/15/they told me that they actually sent the payment to the patient as we are out of network with his particular plan. I was also unable to receive any more information on the claim from ***, the reason I was given was that we are out of network. Thank you,*** ***

***I spoke with BCBS MN and they said that we need to appeal the claim. Which I am more than happy to do. We need you to sign the attached “authorization and release” form. I have reviewed the response made by the business in reference to complaint ID ***, and I look forward to hear if Carole from Immcois able to get BCBS to pay the claim when she speaks to them today.

[redacted]Thank you for including the EOB from BCBS of MN.   I see that they denied due to timely filing, however I did submit proof of timely filing the last time I submitted the bill to BCBS MN.  I will reach out to them today and see if I can get this straightened out with them.I will email you after I speak with them.   Please let me know if you have any further questions.Sincerely, Carole R [redacted] Billing Specialist

I received the attached invoice from  Immco Diagnostics on 4/8/16 with $1,530.00 amount due. Which states “PER BCBS THIS CLAIM WAS PREVIOUSLY PROCESSED. WE DID NOT RECEIVE ANY CLAIM INFORMATION AS WE ARE OUT OF NETWORK. PLEASE FORWARD THE EOB FROM BCBS.”   Attached is the EOB I...

received from BCBS on 1/22/16 which states “THE PLAN DOES NOT INCLUDE COVERAGE FOR CLAIMS THAT WERE NOT RECEIVED WITHIN THE SPECIFIED TIME LIMIT FOR FILING A CLAIM.”    BCBS did not pay Immco because Immco did not submit the claim until 14 months after the date of service which was beyond the specified time limit for filing a claim with BCBS.
