Has been 3 weeks they have not filled out paperwork my job requires now my Job is in Jeopardy.
called 3 times was told not to call again.
Explained to the patient that the form was dropped off on 2/10 and completed on 2/21 and left for him to pick up at the front desk. He received the form and everything was good with his job. He seemed satisfied.
