Patient received treatment.  Patient was not charged for second [redacted].  Also, patient was not charged for [redacted] or reading for [redacted]. Patient has an outstanding balance that has not been paid since 01/09/2014.  Patient has received 16 statements with regards to this balance.   Please call with questions [redacted]

I spoke to [redacted] who did not make an effort to resolve my issue. She offered the same remedy as one year ago which yielded no results. I would like to speak to a manager or CFO.

[redacted]:You were not charged for a 2nd liter of [redacted]s.  [redacted] ordered [redacted].  You were charged for [redacted] and ondasetron only.  NOT for [redacted].As it is, you also did not get charged for the [redacted] to read your film.We tried to call you a couple of times. Thank you[redacted]
