This is in response to # [redacted] I did reach out to Pendrick Capitol Partners and spoke to a Supervisor, Dee I explained the content of the letter the patient received from Central Credit Services, LLC Since the account was sold it is their responsibility to remove the accounts from the credit bureau She assured me that she will be contacting them and sending out a new letter stating that it has been removed I hope this get resolved in the next days Sorry for any confusion and frustration this has caused

We apologize for any confusion and frustration this situation has caused the patient. Unfortunately, our Infinity-Meds billing department did not receive timely information from our extended business office necessary to resolve the issue sooner.Upon review of this patient's account, a
corrected claim has been submitted to the patient's insurance carrier. Our third party agency has been instructed to immediately remove the patient's account from collections and any credit bureau listing.I expect that this resolves the patient concern. If additional information is required, please contact our office directly

Infinity Healthcare called the patients insurance company. They do have our claim but need the patient to call them and tell them which UHC policy is active for the date of service 11/5/16. I called the patient and talked to Mr ***. He will now call his insurance company
and update them on the cob information they requested. Once this is done UHC will reprocess the claim. I also took the 11/5/date of service out of collections and will put the account on hold until the insurance company pays. I also gave Mr *** my direct line if he has any further questions.If you need anything further from us, please let me know. Thank you

Commonwealth Financial collection agency has responded to this complaint.  Since they own the account we can not respond.  I feel their response to be appropriate.  They sent the patient the information she requested.  I believe this matter is taken care of. Thank you

Although we stand by our original position, as a courtesy to the patient, we will close his account with the collectionagency and have it deleted from his credit report. He will receive a deletion letter, in the mail, from the collection agency. Lori [redacted]Customer Service Supervisor.

We last spoke to the Mother on 5/18/16. She has 3 children that had been treated by our physicians. 3 children with 3 different bills. She requested a printout of all 3 bills which were mailed to her, the next day. Each invoice showed all of the payments that she stated that she made in her...

complaint. She did owe all of the money and we do not have a credit on any of the children's accounts. This is in response to complaint #[redacted] dated 8/24/16.  The patient was aware of the 2013 physician bill which is separate from the hospital bill.  We spoke to him several times and told him the VA denied his claim. When the hospital turned in the their bills to the VA, to appeal the...

denial, our outstanding bill was not included.  We do understand that  receiving a hospital bill and a physician bill can sometimes be confusing for the patients.  Since this is back from 2013, and we're unable to appeal to the VA, Infinity Healthcare will write off the balance and remove the account from collections.  We will have the collection agency send a letter to the patient confirming the accounts are closed and deleted from the credit report.  With that being said, it might take 30 days or longer for the collection agency to process this request.If there are any further questions, please let us know.Thank you

To Whom It May Concern,I reviewed Mr. [redacted]’s concern and confirmed that we did
receive his cease and desist letter. However, per Section 803(6) of the Fair
Debt Collection Act, the cease and desist regulation only applies to those who
collect “…debts owed or due or asserted to be owed or due...

another”. This
regulation does not apply to creditors who are billing for their own services.
It applies to third party debt collectors.  Although the FDCPA does not apply to us, we
did honor his request and stop all communication. His account has since been sold to a third party debt
collector.  We have contacted the debt
collector and they informed us that they have not received a cease and
desist letter from Mr. [redacted].Section 805(c) (2) of the Fair Debt Collection Act states
“…if the consumer wishes the debt collector to cease further communication with
the consumer, the debt collector shall not communicate further with the
consumer with respect to such debt, except- to notify the consumer that the
debt collector or creditor may invoke specified remedies which are ordinarily
invoked by such debt collector or creditor. Therefore, the account can be
placed on the consumer’s credit report. Mr. [redacted] cites Public Law 104-208 which refers to
consumer credit debt from credit cards or lending institutions, which again
does not apply to us.

This complaint is an additional issue and associated with my recently-submitted Revdex.com complaint that is specific to a Medical-Debt Collection process. My complaint, today, is a request for further and final action on the same issue identified in my recent Revdex.com complaint. In the previous Revdex.com complaint, referenced above and submitted August 24th, 2016, Infinity provided a prompt answer (in part)-- which at first was thought to have resolved the entire issue. The response from Infinity mentioned that any further question may also be sent to Infinity. Accordingly, the narrative below explains to the best of my ability the related issue which is still unresolved. Accordingly, this complaint is an addendum to my previous Revdex.com complaint. It clearly requires further action by Infinity, known as an original Creditor. This is necessary because the actual issue is NOT fully resolved. It is evident that Infinity wants to resolve the matter for our mutual benefit, and I'm appreciative of the efforts by Infinity to do so. Specifically, a Debt Collection Agency known as Central Credit Services LLC, (CCS), has recently provided a letter to me that identifies the necessity for Infinity to remove any adverse Credit-Report entries from my Credit Report. In contrast to their (CCS) letter, Infinity answered my previous Revdex.com complaint with an indication that the Debt Collection Agency, (and not Infinity), would have the duty to remove any adverse Credit-Report entries from my Credit Report. The recent letter sent to me from CCS resulted from my filing a Federal Consumer Protection Bureau (FCPB)complaint, identifying the failure of CCS to validate a debt. It appears that the entire matter began when Infinity sold an (alleged) debt, reportedly belonging to me, to a Debt Buyer known as Pendrick Capital Partners, LLC. Pendrick, in turn and through several years now, simply assigned that (alleged) debt to Debt Collection agencies. The Department of Veterans' Affairs (VA) has already paid on the matter.TranslateDesired Resolution / OutcomeDesired Resolution:Correction to a credit reportselectDesired Outcome:A correction to my Credit Report, which mistakenly identifies two $810.00 medical billings as delinquent from calendar-year 2014 for Medical services rendered in calendar-year 2013. I do NOT owe the debt. An error in Infinity and St. Elizabeth's Hospital billings resulted in the improper (delinquent) debt reports. The V.A. has already paid my billings in full. This (billing error) is known, and referenced in Infinity's Revdex.com response to me on August 30th, 2016. Infinity letter to Pendrick.

Review: I had an Urgicare visit at St. Elizabeth's O'fallon Urgicare on 06/26/14. The address in 1512 N. Green Mt Rd., O'fallon, IL 62269. Infinity Healthcare provided physician services and billed the insurance company as if the service had been preformed at St. Elizabeth's Emergency Room with the address of 211 S. 3rd St., Belleville, IL 62220. Infinity Healthcare was not paid by the insurance company due to their incorrect/fraudulent address of services billing. I contacted Infinity Healthcare in August of 2014, after recieving a bill from them for $79.86 for physician services and inquiring with the insurance company about non-payment, and informed them they need to submit a corrected billing to be paid as I had already paid the required $40 copay for the visit. They insisted I needed to pay them the $40 dollar co-pay though their bill was $79.86. I again informed them to resubmit the billing with the corrected location per the insurance company. I did not receive any other billing or communication with Infinity Healthcare until 3/2/15 when a letter from a collection company, Americollect, sent an invoice for the 6/28/14 services. I called Infinity Healthcare and spoke with CSR "[redacted]" at extension 221 on 3/4/15 and informed him of the collections letter. He verified that I had spoken to them and reported the billing error on 8/2014 and that they did not resubmit the billing or take any action. He then said the account had sat dormat without any correspondance with me or theinsurance company and they sent the bill to the collections agency. He then said they will not resubmit the bill with the corrected information though I have provided all the information they need to be paid for services.Desired Settlement: I wish that Infinity Healthcare to submit the correct billing with the correct address or to credit the account the amount owed, close the account, and not contact me about this service billing in the future.

Review: We have contacted Infinity on several occassions regarding a billing that is supposedly due. Each time we request for a bill to show what the bill is for they continue to tell us they sent this to outside collections to [redacted] & Partners and cannot provide us a bill to our current address since they sold the account. We contacted [redacted] whom we will also be filing a complaint against because they are telling us that they cannot provide a bill either and we have to go back to Infinity for this. We then received a call from [redacted] Sourcing in Denver, CO who has now taken over the collection for this for [redacted] also stated to us that Infinity has to provide this information due to the HIPPO law and that nobody else can provide this information. Spoke with Inifinity and informed them of this issue and they are still refusing to send us a bill for the services they provided to me on 7/21/12. We moved to our new home in August but we have renters in the address that they sent the billing to and they have also not received any bills for me that they haven't given to me. We also gave all 3 companies our new address. All we want is a bill showing what this was for so that we can pay the bill! Nobody besides Infinity should be giving out my personal information due to the HIPPO law.Desired Settlement: Just need a bill showing what these charges are for for $396.30.

Review: Our son was brought into the emergency room at St. Joe's Hospital in West Bend, WI. with a dog bite to the face. The attending nurses cleaned and bandaged him to stop the bleeding. We were informed that there was no plastic surgeon on staff at this hospital so we opted to take him to Children's Hospital in Milwaukee, WI. As the bills started to come in we required each billing agent provide a detailed invoice, as a condition of payment. All agents complied, and were paid (totaling around $12,000) except for one, Infinity Heathcare Physicians Sc. They provided a bill for $488 for the "services" of the emergency room physician. (*As a side note, their billing comes in a very obscure envelope that does not have their business identified,and it looks like junk or scam mail.) This bill was a surprise to us because the attending emergency room physician, at St. Joe's, did nothing for our son. All the service was provided by the nurses, of which St. Joe's billed us separately. The physician did offered to stitch our son's face but mentioned that he was not a qualified plastic surgeon, so we declined his services and had our son transported to Children's Hospital. Now, I come to find that Infinity has sent our bill to collections. This is all so exhausting for the consumer.Desired Settlement: Remove the bill from collections, provide a review of the charges to determine if they are ethical. Provide a letter stating that the charges have been removed from collections. Adjust the bill to reflect an ethical charge for actual services rendered. PROVIDE A DETAILED INVOICE OF THE CHARGES sent in a envelope that clearly identifies "Infinity Heathcare Physicians" as the sender. We cannot accept a billing statement that simply states emergency services - $488. We need an invoice that give us a detailed description of what the physician did and the cost associated with each of his services. At which time we will review the invoice and contact Infinity with any remaining questions.

Infinity Healthcare does take patient concerns seriously and would like to address and resolve the concerns regarding [redacted]'s account. This account and medical record has been reviewed by the Billing and Coding Managers. An itemized statement was provided previously and several phone calls made in an attempt to address the parent's concerns. Infinity will be happy to provide an additional statement which will reflect an adjusted fee for services provided. [redacted]'s account has been removed from third party collections and a letter confirming this will be mailed from the agency.

We will promptly address any additional questions the parents have once they have reviewed these documents.
