We are in receipt of the complaint made by Mr [redacted] relating to the following dates of service: 10/3/16, 11/7/and 12/16/ I understand that Mr***’s insurance company denied the claim because incorrect patient information was submitted by Ingalls I am very sorry that the patient had difficulty resolving this matter with our Customer Service Department Please be advised that Ingalls has made the corrections to this claim and rebilled Mr***’s insurance carrier as of 9/5/Additionally, we have removed these accounts from our collections process and as such, he should not receive any more telephone calls related to collections for these dates of service Again, I would like to apologize for the difficulty and inconvenience Mr [redacted] encountered with our Customer Service Department I can assure you that Ingalls has taken steps to improve this process for our patients going forward Additionally, we would like to extend a discount on Mr***’s remaining balance due for these dates of service It is our hope that we can resolve this matter to Mr***’s satisfaction and that he will continue to choose Ingalls for his health care needs

The complainant states she has not yet received a bill for services, despite multiple requestsWe sincerely apologize for the error and will waive the balance as a courtesy for the inconvenience this caused Ms***A zero balance statement will be sent within the next business days to Ms*** from Ingalls via a traceable method to the address Ms*** provided to *** *** on May 14, Further, we will notify the credit reporting agency that this account should be removed from the patient's credit report without negative impactMs*** can expect to receive a letter from the credit reporting agency within approximately business days confirming that this action has been completedAgain, we apologize for the inconvenience and hope that this resolves the matter to Ms***'s satisfactionMs*** may contact *** *** at XXX-XXX-XXXX with any additional concerns

29, visit to Ingalls Memorial Hospital Family Care Center in Tinley Park ("Ingalls")Our investigation shows that Ingalls never received a charity application from Ms***Further investigation shows the employee Ms*** was attempting to contact ("***") is no longer employed by Ingalls Memorial HospitalVellar's voicemail should have reflected this and her calls directed elsewhereI am very sorry that did not happen, but can assure you that issue has since been correctedHowever, when Ms*** was directed to *** Smith, it is documented that her call was returned on 7/by Shalon ***, who left a voicemail message stating she was returning the call on *** *** behalfIngalls would like to ask Ms*** to send a copy of the charity application that she submitted previously, so that we can review it and complete her requestMs*** may fax the application to XXX-XXX-XXXX and ask that she follow up by phone with Shalon *** at XXX-XXX-XXXX at her earliest convenienceI would like to sincerely apologize for the service Ms*** received, but can assure you we are looking forward to speaking with her so that we may address and resolve this matter expeditiously
