Dear [redacted] , We are in receipt of the complaint filed through your office by one our client/patientThe ID ref from your file is [redacted] ( [redacted] )I sent you a fax response last night as soon I received the scanned letter you sent to attention of [redacted] at our Redlands clinicTo address the issue further, I have more input from the Moreno Valley clinic staff – see office manager response below Based on the account given below from office manager and our office procedures, I understand that there is delay in processing medical records request for the patient due to the holidays and the treating physician not being in the officeGiven the nature of our patients, medical records are strictly under the control of treating physician’s discretionThere are times that the records are not released to patient but to only another treating physician or to an attorney if the patient gives written permission The staff tried to explain the situation and yet the patient was not clearly happy about the delayI tried to call and speak to the patient directly today and the call was not answered and so I left a message stating that the records would be ready by tomorrow evenning or the day after once the physician returns from vacation and that she would get another call from the staff once the records are ready for her to pick up From the call logs and voice mails the pt called once on 26th and twice on 27th leaving messages stating that if she can’t get her records she would file complaints with every board, etcHowever, the office was closed on the 26th and pt did not answer or reviewed her voice mail when she came to office directly as well Please read the message below from office manager in Moreno Valley to understand the situationIf you or the patient have more questions you can reach Susan [redacted] at ###-###-#### or myself directly at ###-###-####Please feel free to let us know if you have any further questions and or feedbackSincerely, [redacted] From: [redacted] [mailto: [redacted] [email protected]] Sent: Tuesday, January 9, 9:AM To: n***@socalpsych.com Subject: re: FW: Revdex.com complaint on IPMG Hi ***, Yes I talked to this person and explained to her that the doctor was out of town and we had to wait for his signature on the MRRQShe just wanted to yell at someone in the office and when I asked her to stay so I could go back and check on the status of her MRRQ she stormed outWe have record of her only attempting to call one dayAfter the one day she came into the office and stormed out without waiting to be helpedLater that day [redacted] did call her to discuss with her the MRRQ statusThere was no answer, so she left a voicemailWe did not receive a call backWe were not notified that the patient was attempting to contact the Redlands officeIf you check the patient quick notes the encounters were documented on 12/28/If you check the tasks it was sent to [redacted] on 12/towards the end of her shiftBy the time [redacted] had gotten to returning voicemails from the previous day, that patient was already in the office From: n***@socalpsych.com Sent: Monday, January 8, 10:PM To: [redacted] [email protected] Cc: " [redacted] " < [redacted] @socalpsych.com> Subject: FW: Revdex.com complaint on IPMG [redacted] , Pl find out if any of the staff called the pt and explained that the Dr is out of town?
