In this situation as stated by the claimant the estimated amount due was $225.00 which was paid.A statement was generated to the patient which prompted a call according to our records on 7/5/2017. It was found that the payLinent was on record at the clinic facility however was not found on file in the billing records. The account was escalated to determine the whereabouts of the payment. Unfortunately the payment was not located until 9/21/2017. It was found that the payment had been incorrectly applied.On 5/18/2017 the insurance company processed the claim to the claimant’s out of pocket expenses in the amount of $195.81. This created an overpayment in the amount of $29.19 which was refunded to the claimant on 9/21/2017. Payment should arrive by 10/2/2017.We apologize for the inconvenience.

In this situation as stated by the claimant the estimated amount due was $103.00 which was paid. Per the [redacted] health eligibly system the plan has a 20% coinsurance for a MRI. Documentation from the [redacted] website is attached.As noted by the claimant a refund of $92.75 was made after the health insurance processed the claim for payment. The insurance processed the claim and allowed $515.00 and paid $504.75 leaving a coinsurance balance of $10.25 which was not refunded. This information was obtained from the electronic admittance advice received from the insurance carrier, copy attached.As a result of the Revdex.com complaint we further investigated the co-insurance balance of $10,75. It was found that this amount in not an insurance co-pay rather a service charge imposed by the clearing house that processes electronic payments on the behalf of [redacted]. It is not our company practice to pass these service charges imposed by health insurance companies on to our customers. We will be refunding the $10.75 to the claimant.
