Dear [redacted] I am in receipt of your letter informing our business of a complaint from [redacted] . I wish to respond to her complaint. Ms [redacted] did initially come into our office seeking [redacted] . Our office policy and that of medical staff requires all patients seeking [redacted] ... must receive nutritional counseling sessions with the medication as well as a month supply of B-12 injections. At the time of the sale Ms. [redacted] was informed of this and was also informed that she must meet certain medical qualifications before being prescribed the medication. The cost of the medical professional visit was explained to her as well as the cost of nutritional counselling sessions. She was then scheduled for a follow-up visit with the CRNP. Upon the visit with the CRNP Ms. [redacted] was disqualified medically for the [redacted] . At that time she did receive and take the B-12 injections. Ms. [redacted] also signed a sales agreement stating that once medication is taken out of the office it cannot be returned.When Ms. [redacted] requested the refund on August 12,2016 of the nutritional counseling sessions and [redacted] she was informed that it does take 30-60 days to process a refund. She since has been refunded on $145 September 15, 2016, which is 2 nutritional counseling sessions at $50 each and $45 for 1 month of [redacted] . In regards to our conversation yesterday morning we are providing a statement from Greater Pittsburgh...

Medical Weight Loss in response to Ms. [redacted]'s complaint #[redacted].
Greater Pittsburgh Medical Weight Loss [GPMWL] takes great care in providing our client's with excellent customer care, but, as any business, we have to collect compensation for our services rendered and products sold. It is our position that Ms. [redacted] was pleased with the service and the products, so much so that she had referred two others to take part in the program (as seen in the provided text message log date July 8th). However, when Ms. [redacted] was alerted to the fact that her account balance needed to be brought current, that products and/or services would be refused until the account was paid down, reminded that a cancellation of the contact would incur a $200.00 cancelation fee (as stipulated in both Service Agreements). Ms. [redacted] elected instead to no-show for appointments, stopped payment on her balance and proceeded with filing a baseless complaint with the Revdex.com requesting a full reimbursement for services rendered and products sold. 
We will first address Ms. [redacted]'s key points in her statement to the Revdex.com and then provide evidence to the contrary. 
Point A:
"My first visit I payed over 300.00 for a doctor visit who I never saw and blood work…I end [sic] up getting my shot 4 to 5 days after I paid for the program"
Ms. [redacted] was provided a blood work service as this is standard protocol due to the fact that no appetite suppressants can be prescribed without first having blood work done and reviewed by one of our medical doctors. The procedure was completed on 05.12.2014 and, after a review of Ms. [redacted]'s blood work, medical appetite suppressants were prescribed to her. The time delay on receiving the first injection [3 days after the initial consultation] is because Lipovite injections are not dispensed on the first visit. The delay on the medical appetite suppressants are a direct result of having to have blood work reviewed by a medical professional prior to receiving medical treatment. In addition, Ms. [redacted] did work with a licensed nurse practitioner, [redacted], CRNP, on three occasions [05.14.14, 06.20.14, 07.18.14] to be instructed on procedures for administering any injections as well as to guide her through the program. Regular checkins and weight loss counseling was then provided through one of our weight loss counselors. 
Point B: 
"I did not get a contract at all not [sic] was I told that if I break the contract there would be a 200.00 fee..Nor was I told I was locked in.."
Ms. [redacted] is simply mistaken. Attached to this email is two Service Agreement contracts which were signed and initialed by Ms. [redacted]. The contracts detail not only her initial package plan, but an update to the agreement to add additional services dated 05.07.2014 and 06.20.2014 respectively. At no point did Ms. [redacted] request copies of her signed agreements from GPMWL and instead insists that she was never instructed on her obligations which is patently false.
Point C:
"My car broke down [sic] I did not have a car for 3 weeks [sic] I called and told her my situation she never mentioned about my balance or the contract" 
Again, Ms. [redacted] is mistaken. On September 9th she was reminded via text messages with her counselor, [redacted], that she needed to setup a payment plan with her account or bring account current and had answered affirmatively that she had received these text messages. Finally, [redacted] instructed her that GPMWL could not dispense any more products or services until the current balance on the account was taken care of. 
Since Ms. [redacted] stopped communicating with her weight loss counselor, the collection on the account was handled by the Account Coordinator, [redacted]. Ms. [redacted] made several calls and emails to [redacted] (attached), after hours, to which [redacted] responded requesting the account to be brought current, communicated GPMWL's interest in working with Ms. [redacted] to settle her account and even offered to setup an auto-draft or payment plan. Additionally, Ms. [redacted] was alerted during these correspondences to her agreement that termination of the contract would result in a $200.00 fee. 
Prior to abandoning her program with GPMWL Ms. [redacted] repeatedly contacted our coordinators and counselors after normal business hours and on Sundays despite being reminded to call back or contact GPMWL during operating hours. In some instances GPMWL representatives could assist her, in other instances they could not because it was outside of normal operating hours. Our email and text communications with Ms. [redacted] can be viewed via the attached documents. Furthermore, Ms. [redacted] repeatedly missed scheduled appointments citing impromptu trips to Atlantic City and scheduling conflicts. However, Ms. [redacted] had agreed [on both contracts] to the stipulated $25.00 cancelation fee for missing appointments. In total, she has missed three (3) scheduled appointments. As a result, these fees, which were explained and initialed by Ms. [redacted] in the attached service agreements, will be added to the balance on her account.
We are understanding Ms. [redacted]'s request for full reimbursement via the Revdex.com as her willful termination of her contract with GPMWL. Currently, Ms. [redacted]'s account balance with GPMWL is $875.00 and is detailed on the attached Excel report. Included in this account balance is the previously stipulated $25.00 appointment cancelation fees, the agreed upon $200.00 fee for termination of the contract and the total balance on the account including services rendered and products sold.

Review: In May 2014 I decided to join Greater Pittsburgh Medical Weight- Loss Program.. It detailed B 12 shots vitamins and eating suppressants. I contacted them made a appointment not realizing how much of issue this was going to be.. My first visit I payed over 300.00 for a doctor visit who I never saw and blood work in which I was told I did not need at all then they told me it would be best after the rep asked and I was told not to worry because I really did not need one... I end up getting my shot 4 to 5 days after I paid for the program now durning this time I did not get a contract at all not was I told that if I break the contract there would be a 200.00 fee.. Nor was I told I was locked in.. I called the head office and talk to [redacted] not sure what she is manager or owner I told her the rep that worked with me did not teach me nor explain to me the process of this plan.. She signed me up took my money and told first not to worry about blood work then tells me I need it.. A friend of mine had to explain to me about the shots and how to inject them myself.. The rep name was [redacted] she end up being fired.. I never got a call nor an apologize from this company for all I went thru.. After about a month my supply was up I called to reschedule took 2 days for them to call back to set an appointment.. Again I was not told about the contract or the fee of 200.00 if I break it.. Nor was I told how long I was in this program for.. I never got a contract!!!!! I was told I can pay as I go and not to worry I put 150.00 got my supplies and was told to call or text.. So I got in touch with the rep explaining that I need appointments but it had to be after 5:00 because of my job she had appointment at 4:00 I can't be there because of my job.. My car broke down I did not have a car for 3 weeks I called and told her my situation she never mentioned about my balance or the contract.. I texted her a few weeks ago telling her I wanted to come in to pay get supplies and set up a payment plan I explained to her that I can't talk at work we have a no cellphone policy and I feel that texting is not appropriate and I asked her when it would be good for me to call her... No response back.. I got a call from [redacted] on Monday stating she needed to discuss my balance I called her after hrs I work till 600 or 700 at times.. No call back.. I had to email the company that is the only way she communicated with me.. She kept on telling me how the contract would be broken and I had to pay 200.00??? What contract I didn't receive anything.. I don't even know how long I am in this program for??? I extremely upset because after all this company put me threw not once did the apologize your try to compensate me at all.. I don't understand about this contract because I don't have a copy of it!!! Nor did I know about a 200.00 fee now they want me to pay the balance off and continue with the program??? The first rep took my money I had to wait 4 to 5 days to get my supplies which should have been given to me once I paid.. [redacted] never spoke to me about this.. I was told I can pay as I go??? Never did the send or contact me about my balance until I called to go into the office for supplies and to pay some of my bill???Desired Settlement: I want a refund for all the money I spent on these products.. They lied and lured me into believing that they work with me and they have not.. I work hard for my money and I don't appreciate being used..

Review: I was billed in advance for Nutritional Counseling Sessions, B-12 shots (that I didn't want but they stated I must pay for and use if I was to get prescribed [redacted]), and an [redacted] prescription. The total costs of this were over $300. I was called to come in to the office and informed that they were waiting on the Dr. to complete the paperwork for the [redacted] but that I would be able to pick it up the next day. This same day, they provided me with the B-12 shot injections that I stated I did not really want (I have a fear of needles) but would give it a go since it was a condition of using the [redacted] that I was to receive the next day.

The next day (a Friday), I received no calls, the following week, I had to call them only to get a return call stating that I was not actually approved for the [redacted], and then told that someone from finance would be in contact with me to issue me a refund.

This did not happen, and despite numerous calls (at least 5) and promises to be refunded and called back, I have not received any refund or calls returned for over 2 months now.

This is really a bad practice. I would like to be refunded and strongly recommend that this company only bill for services actually rendered.Desired Settlement: Full refund, as I have not had any services performed outside of taking the b12 shots, which I only took to fulfill the requirement of taking in conjunction with the [redacted] & counseling that was promised but never received.

Ms. [redacted] also signed a sales agreement stating that once medication is taken out of the office it cannot be returned.When Ms. [redacted] requested the refund on August 12,2016 of the nutritional counseling sessions and [redacted] she was informed that it does take 30-60 days to process a refund. She since has been refunded on $145 September 15, 2016, which is 2 nutritional counseling sessions at $50 each and $45 for 1 month of [redacted]. Review: I had my first appointment with Integra Medical in April. I came to the medical group ONLY to see their Weight Loss counselor Cassandra. I had heard WONDERFUL reviews about her and she is the only reason that I chose this practice.

She first put me on a weight loss plan that did not work for me, as my body did not respond to that type of injection. She just started me on a new program and I had a follow up appointment scheduled next week (6/6/16 @ 11:30am.) I had planned on driving to Robinson to see her because she did not have any availability at their Monaca location. I met with her weekly and she would weigh me and do my measurements and BMI.

I received 2 confirmation calls from Integra to make sure I was still coming (I have never received confirmation calls in the 2 months I have been going there) so I thought it was odd. I called back and asked if I could speak with Cassandra because I had a few questions regarding my diet change and the new injections I would be picking up next week. The lady at Integra told me that I would have to talk to Elise or Tina about it. A few hours later I received a phone call saying that if I did not want to drive to Robinson that I could do an email/text visit or a face time chat visit. It doesn't make any sense to NOT go to the office, how would they weigh me, do measurements etc. through email? THIS PRACTICE IS A JOKE.

Come to find out through a friend - Integra Medical ended their contract suddenly with Cassandra and she was let go on 5/26/16. The medical group has not informed me that I would no longer be able to meet with Cassandra.. I have left several messages yesterday/today to have a manager call me back and I have not heard from anyone. I would like a refund for the services that I was supposed to have with CASSANDRA. As I have no interest in meeting with Elise or Tina. I chose this practice solely because of Cassandra - If she is no longer employed by Integra I would like to receive a refund as I will not be receiving services from the practice.Desired Settlement: I would like to receive a refund as I will not be receiving services from the practice.

Review: This organization does not give you what you pay for and are very unresponsive. Had to email numerous times before I received a response. Currently disputing charges of $1,600, which they now tell me there is a value of $2,147 and are only going to refund $162.

I would never recommend them to anyone!Desired Settlement: I would like more than a refund of $162. They are terrible to deal with and very unresponsive.

4/27/2016To whom it may concern:In regards to complaint id# [redacted], I wish to file a formal response to Ms. [redacted]’s complaint. Soon after Ms. [redacted] purchased a weight loss program with our office, the doctor, Dr. S[redacted] became unavailable to see patients in our office. Our sales person gave Ms. [redacted] another weight loss program in place of the original plan she was on, in addition she given $499 in additional supplements at no cost. However due to the fact that the original plan she purchased was not available I offered Ms. [redacted] a refund of $759 that was credited back to her Care Credit account.
