This response is in regards to the letter Integrative FamilyMedicine Associates (the "Practice") received from Revdex.com ofEastern MA, ME, RI & VT' ("Revdex.com"), dated October 9, 2017, andreceived on October 14, (the "Letter")The Letter alerted thePractice to a complaint filed by *** *** on
September 26,The complaint alleged that the Practice repeatedly failed tosend the vaccination records of *** ***'s daughter (the"Patient") to his home address, and to the office of the Patient’snew primary care provider ("PCP"), *** ***.The Practice unequivocally denies the allegations in the Letter,and, in response, provides Revdex.com with following timeline of events:• The Patient visited the Practice for appointments on 5/24/2016and 7/18/2016.• As is our customary practice, Patient's parents were providedaccess to our online portal, which allows our patients, or theirguardians, to view all of a patient’s clinical records.• On January 3, 2017, Patient's mother informed the Practice thatPatient's care was transferred to *** ***, their newPCP.• The Practice requires a written release authorization to be on file,and charges a twenty-five ($25) Dollar fee per record beforereleasing any medical recordsThis information is availableon the Practice's website along with a downloadableauthorization formThe Practice may waive the fee oncertain occasions.• After receiving the authorization form from *** ***'soffice, at *** *** (the "Office"), Practice waivedthe fee and faxed the records to the Office.• Since the Practice did not receive any indication of non-receipt of medical records or further requests to re-send medicalrecords from the Office, which usually occurs when aphysician's office does not receive the expected records, thePractice assumed receipt of Patient’s medical records by theOffice.• On September 13, 2017, the Practice received a call from *** requesting that we send the Patient's medical records tohis new home address.• On September 17, 2017, the Practice mailed Patient's medicalrecords to the home address provided by *** ***.Even though the Practice already faxed and mailed Patient'smedical records, in an effort to expeditiously resolve this matter,the Practice contacted the Office, faxed Patient's medical records,and confirmed receipt of sameAdditionally, the Practice sentPatient's medical records to *** ***’s home address onOctober 20th, 2017, by certified mail, return receiptrequestedThe Practice is currently awaiting a return receiptindicating that the Patient’s medical records have been received by*** ***
