laboratory services.Ms *** has had multiple services and claims with us since Unfortunately,Laboratory billing systems are set up to treat each patient encounter separately We do not setup permanent patient files, like physicians do, with basic demographic information.A system process overrode the address on her September claimAfter her payment was received, we corrected the address to avoid future errors in mailing her invoicesThis system process is being changed in the very near future to a/levi.ate future issuesWe sincerely apologize to Ms*** for this process that automatically changed her record to an older address.For a claim, after not receiving any payment for four months and several mailings, DLS sent her claim information to our collection agencyAfter sending the claim for collection, the collection agency contacted Ms*** and confirmed on the phone that the mailing address DLS provided was correctAfter multiple calls and messages, no payment was received in daysAt that time, the agency put notice on her credit report regarding the non-paymentWhen her payment was received in May 2012, the agency removed this notice from her reportAlso, Ms*** downloaded from the collection agency web site, two (2) Letters of Deletion that may be used to obtain a rescore of her credit report.*** volume does not allow us to call patients for each outstanding paymentWe do ask the Patient Service Centers to confirm the address on the order form with the patient at the time of services We also try to find correct addresses when mail is returned to us.Again, we offer our apologies for any the inconvenience she encounteredShould you have any further concerns, please feel free to call me at *** ***

After her payment was received, we corrected the address to avoid future errors in mailing her invoices. This system process is being changed in the very near future to a/levi.ate future issues.
