Review: I signed up for health insurance through my company, [redacted], inJanuary of 2014. My premiums are being taken out of my checks on a by-weekly basis. As of today, 02/10/2014 I have not received my medical, dental or HSA cards in the mail. I called on three separate occasions to a [redacted], the week of 02/03/14 to 02/7/14 and left several messages with [redacted], as of today I still have not received a call back from her. I believe this is unacceptable and very unprofessional practice and service.Desired Settlement: [redacted] needs to call her clients back quickly and professionally. I would like my cards that I am paying for to make necessary appointments with my health care providers.

We received two phone calls from Mr. [redacted] on 2/4 and 2/6. We spoke with the benefits administrator who is the responsible party at Mr. [redacted]’s place of employment for administering online enrollment. The benefits administrator is an employee at Mr. [redacted]’s place of business and is responsible for ordering and distributing the ID cards to the employees for the benefit plans offered by the company. The benefits administrator at Mr. [redacted]’s place of employment ordered new ID cards from United Agricultural Benefit Trust (UABT) for him on 2/6. On that same day the benefits administrator printed a temporary ID card from the UABT website as well as included a copy of the Assurant Dental ID card and sent both through their interoffice mail truck overnight. Mr. [redacted] should have received the card by 2/7. The benefits administrator contacted Mr. [redacted] to let him know that they did not have a Health Savings Account (HSA) but rather a Flexible Spending Account (FSA) and that there would not be an ID card for the FSA plan.
