Also enclosed is the explanation of benefits from Mr. [redacted]'s insurance company. As you can plainly see they paid absolutely nothing on Mr. [redacted]'s behalf.
Please direct your attention to the first paragraph of the financial policy where it states that any portion of a patients bill not covered by his/her insurance is the patient's responsibility, In this case everything!!!!
