From: Jeff Y*** Date: Wed, Dec 14, at 7:AMSubject: Complaint ***To: "***@cleveland.Revdex.com.org"
Good morning,
I'm responding to this complaintFirst of all, the prescription was followed as written. During the
complaintants registration process, he signed a form that specifically states that there is no guarantee regarding the outcome of the treatment. I would be happy to fax a copy of the signed form to you.
During his first visit, he showed me a pair of custom made foot orthotics (arch supports) that had a certain component called a ***'s plate incorporated into them. He indicated that these were provided by a podiatrist a few years ago. Prior to seeing me, he went to another facility to get new orthotics. They were unable to satisfy him and he was very unhappy with them. I told him at the time of his first visit that the only ***'s plates I had or had access to were more rigid than what he had. He still requested that I proceed. I attached the rigid ***'s plate and asked that he return for a follow up visit in a few weeks. When he returned, he indicated it was too rigid. I removed the rigid plate and made a plastic ***'s plate that was more flexible than what he had on his original orthotics and attached it to his foot orthotic and asked that he try it for a few weeks and return again for follow up. When he returned, he stated things were ok so I glued the plate to his orthotic. A few days later he called the office demanding a refund. He was told that the devices were custom fabricated for him and he had office visits as well and we would not issue a refund
He went to a doctor (a different doctor than the one who wrote the prescription). The physician sent a letter requesting that I attach a ***'s plate like the one he had. I called the doctor and explained that I could not locate a plate like he had and the doctor requested I do the best that I can, which I did
The patient came in for another appointment asking if I received the letter. I told him that I had received the letter and explained to him again that I couldn't locate a plate like he wanted so he demanded a refund of his money. We refused to issue a refund based on the information above
In summary, the complaintant was informed in writing as well as verbally that there was no guarantee regarding the outcome of his care. He was never told he would be refunded his money if he wasn't happy with the outcome. Our company incurred costs in fabricating the custom made foot orthotics as well as the cost associated with his office visits. Our company has been in business since January of and to my knowledge this is the first complaint ever filed against us to the Revdex.com
Please confirm that you have received this response. Thank you.

How many times do I need to repeat thing. He was shown a sample of the orthotics before we began. He was told there was no gaurentee regarding results of treatmentHe acknowledged this in writing by signing the permission treat form informing him of this. He was told this verbally by me as well. No one ever told him his money would be refunded is he wasn't satisfied. I have no idea where he ever got this idea. He is obviously as confused about his treatment as he is about his payment mechanism. We have in our possession a "dispute notification" from a Visa health care prepaid card that the account holders card has been credited on 12/5/16. Again, as with other forms I'd offered to fax over, I'd be happy to fax a copy of this as well. I no longer wll respond to any of his further comments

We called the customer [redacted] Finan on 12/4/2016 and left a very detailed voice mail and instructions to call us back.  We have not received a call back as of yet.  We will under good faith pick the machine up, repair the machine and return the machine. At that point we will consider this issue 100% settled. We cannot go forward until we hear back from this customer. Randy M[redacted]CEOVapamore
