Tell us why here ID# [redacted] This review is based on chart entries: In August 2014, the patient presented with pain on the lower rightUpon examining the area, it was evident that she had decay under two old crownsAt that point, I referred her to an endodontic specialist to evaluate the vitality of the teethThe specialist confirmed that there was decay on both teeth and there was a possibility that one of the teeth would need a root canalThe other tooth, which already had a root canal, may need to be retreatedThe first step was to remove the old crowns and excavate the decay in order to determine the best treatment optionsIt was determined that both teeth could be restored, in which I had placed new crowns on both teeth after the endodontic treatmentFour months later, she presented with one of the teeth fractured to the gum lineAt that point, I referred her to an oral surgeon to remove the tooth and to evaluate the site for a possible implantI informed the patient, that since it was a recent crown, that I would not charge her for a new crown that would be placed on the implantHowever, I did tell her that I would charge her for the custom abutment that would be placed under the new crownThat was the last time I saw her Since she is being treated by another dentist, I would be glad to write off the balance she owes for the crown that fractured offI will be doing this not because I feel that the treatment was inappropriate, but that it would be fair to the patient Sincerely, JMichael H***, D.D.S

vitality of the teeth. The specialist confirmed that there was decay on both teeth and there was a possibility that one of the teeth would need a root canal. The other tooth, which already had a root canal, may need to be retreated. The first step was to remove the old crowns and excavate the decay in order to determine the best treatment options. It was determined that both teeth could be restored, in which I had placed new crowns on both teeth after the endodontic treatment. Four months later, she presented with one of the teeth fractured to the gum line. At that point, I referred her to an oral surgeon to remove the tooth and to evaluate the site for a possible implant. I informed the patient, that since it was a recent crown, that I would not charge her for a new crown that would be placed on the implant. However, I did tell her that I would charge her for the custom abutment that would be placed under the new crown. That was the last time I saw her.
Since she is being treated by another dentist, I would be glad to write off the balance she owes for the crown that fractured off. I will be doing this not because I feel that the treatment was inappropriate, but that it would be fair to the patient.
