Westgate SmilesCentral AvenueSuite B4Albany, NY 12206Please see attachments and refer to ID#: [redacted] If you have any further questions, you may contact our office manager, Keri.P: [redacted] *Email: [redacted] Thank you[redacted] came to our office on 12/03/for a consultation to fix her teethDiagnosis and hertreatment plan were discussed in detail with [redacted] was aware that her insurance company doesnot cover immediate dentures and she did not want to wait weeks without teeth.On 12/23/2014/***'s upper arch of teeth were extractedOn 12/24/2014, all of her lower teeth wereextracted and impressions were made for her Full upper and Lower dentures.On 12/31/2014, [redacted] had come in for a Wax Tof her teeth set up in wax to make sure she likesthemOn this day, it was clearly explained to [redacted] that because of her bone structure and naturalanatomy it was not possible to push the teeth inwards and because she is getting a removableprosthesis it will add fullness to her face and she understood [redacted] requested to try smaller size teeth.On 01/07/2015, another Wax Twas done in smaller sized teeth as requested and the protrudedanatomy ifthe bone structure was explained again to her [redacted] approved the look and esthetics of herdentures this day and only then her dentures were finished.On 01/08/2015, ***'s Full dentures were delivered with verbal and written post insert instructionsand were advised to come back for adjustments.On 03/09/and 04/27/2015: Adjustments as needed and a soft reline were done to help fit thedenturesAll adjustments and the soft reline were not billed to the patient or the insurance and weredone free of charge.It was explained to [redacted] that she is still healing and her gums are shrinking and will continue to do soup to 5-months after extractionsThe adjustments are needed and are to be expected [redacted] wasinformed in June that she will be covered by the insurance to get a realignment/hard reline of thedentureThis was explained to ***, she was informed that the step will help and improve the fit and itis very important that she come back for that step[redacted] never returned to Westgate Smiles for any further help she might have needed [redacted] made acomplaint to her insurance as wellWe have presented all the details to her insurance companyThecomplaint was dismissed because we complied with all the regulations and each and every step wasdone in the most accurate way and was thoroughly explained to [redacted] as well.We are still willing tohelp [redacted] with her dentures and do whatever we can only if she comes to the office.I would like to take the opportunity to assure you that all the treatment was done in ***'s bestinterest and with her consent at every step, following all the rules and guidelines of the book andinsuranceWe have tried to explained this to [redacted] on multiple occasions.If you have any further questions, please feel free to contact our officeThank you,DrTanvir S***

The patient came into our office as a new patient in which she had an exam and x-rays completed on 05/02/On her first visit, she presented Westgate Smiles with a Groupon voucher for $The Groupon voucher covers an exam, a prophy, and x-raysAlana came back on 05/31/for a prophy
However, the front desk mistakenly did not post the Groupon payment that was made by the patient, therefore, resulting in a balanceThis was a mistake on our partWe have adjusted the patient's balance to zero and have also sent her statement to the address she provided us withWe apologize for the confusion and the delay
