Our standard communication is that with dual insurance, as a courtesy to our patients, we will bill both insurances for the full amount and do our best to get them to pay We also caution our patients that there is no guarantee that rendered treatment will be fully paid even with dual insuranceBecause of the nature of verbal communications with patients, we as a practice are diligent with obtaining a signed acknowledgement of potential charges at the time of treatment In this case, at the time of treatment, we did obtain a signed Financial Arrangement Form; if a copy if needed we will be more than happy to provide it.As far as her medical records are concerned, we are happy to provide them to her In order to do so, per HIPPA regulations, we will need a Signed Consent to Release Medical Information form that will allow us to release them.Thanks,***-- [redacted] A+ Dental Care [redacted] ***

We take great pride in providing quality and caring dentistry for all of our patientsI know that we will not please everyone but we have never forced treatment upon any of our patientsWe are sorry that this patient is unhappy with our service but I was not able to see any notes that she had come into the office and informed the staff that she was unhappy with the treatment nor that her teeth were in painIf this is still the case we have no problem seeing her to figure out why she is having these issues

I am rejecting this response because: The signed document is from 2010...where are all the documents I had to sign in that included my current address???? I still firmly feel that my account should be resolved with out payment due to the subpar business practices of this establishment and I STILL have not received a bill as of last night. I've searched my email account and have zero record of any email contact from this office on 8/9/16. I'm completely disheartened at this establishments business practices and the fact that if I was deceased - Dr*** only concern was her payment and she approved sending a deceased person to collections. If that does not speak to the level of basic human disregard and lack of ethics nothing does. This matter is so much more about professional business practices and not the money. I am awaiting a response from the California Dental Board regarding the billing practices because Dental practices must have to follow some established rules. I'm also checking into the fact that the office took payment from my credit card several days before the service was performed - poor business practices

I would like to address some of the patients concerns: #We provide an estimate for services to be rendered prior to the work is completed and billed to patients insurance company. When dealing with insurance companies we never guarantee this will be the exact payment that their
insurance will pay as the insurance companies do not provide a guarantee of payment. Therefore that is why it is referred to as an "Estimate". #For the address/mailing information on file this was what she had provided us back in and we do not show we have any updates to any other information. So all the bills are send to what we have on file. We did receive the statements back from the post office with a written note "Deceased" on it. (which I have provided a copy of that returned mail) #We submitted her charges to her insurance company the day after the services were provided (4/29/16) and received payment from insurance on 6/2/16. Therefore no bills would have been sent to her until after her insurance pays. We do ask for payment on the first appointment especially if patients are going to be under any type of meds prior to the appointments. The first statement was issued on 6/8/for what was remaining after the insurance payment. I have also provided a copy of the explanation of benefits.The 2nd statement was issued on 7/13/16. The 3rd statement was issued on 8/12/along with an email sent out to try and contact patient to discuss. Email was sent out of 8/9/and then a phone call was attempted on 8/11/16.Again I have enclosed those notes. #On the note that we may be potentially doing illegal balance billing to a patient is untrue. Again we provided an estimate for services and what the insurance does not cover the patient is responsible for which is also noted on the estimated signed by the patient. I have also enclosed a copy of our Financial Policy that was signed by the patient.Please let me know if you have any questionSincerely,***Front Office Manager***

I wish they clearly mentioned this that they will bill as a courtesy and they will not coveredBecause when we went they mentioned it's will be covered 100% and we don't need to pay anything that's why they pushed so muchThey got me signed saying they will bill that portion to the second insurance and it will be paidThey even called to clarity that one tooth will be covered 100% that's why I said I will do one.*** insurance said the dental office knows that the second insurance will not payWhy they didn't mention thatIf I did one tooth it will be less problem, but doctor forced me to do saying I made this appointment to do three. Patient came into the office for a teeth cleaning. She received a teeth cleaning along with an exam from Dr. [redacted]. They did inform patient the need of treatment but no work was done on the patient during this visit. She has stated that "they have removed my fillings in my mouth top and bottom...

without my authorization. I was not having any pain in my mouth at all." We have tried to contact patient via email letting her know that we did not remove any fillings from her teeth and the last time we have seen her other than for cleanings was back 2016 and that if she wanted to come into the office to discuss we would be more than happy to meet with her.
