Review: I went to Dr. [redacted] for a bridge replacement, my dental work with a temporary bridge was placed, then I agreed to pay 500.00 dollars down and 375.00 dollars per HSA per month. I have now been informed that Dr. [redacted] wants 50% of the bill up front, which has not been discussed until today and that He will not send my mold for perment bridge until 50% paid, That I should be happy with the temporary bridge he gave me, and "what did I expect a free Bridge?" Dr. [redacted] Manager, [redacted] told me that I must pay before they will finish my bridge and acused me of wanting my bridge repaired for nothing. Their process failed, if I would of been informed that my payment agreement would be cancelled and that my integrity would be attacked I would never of had the work done. [redacted] insisted that I take out a credit no intrest loan/card that Dr. [redacted] holds and make payments that way. When I explained that legally, I am unable too, again I was condsenedly talked to as if I was stealing from them. Now, instead of a real bridge, I have a temporary bridge and it will take one year to reach the 50% that Dr. [redacted] demands before completing my procedure. I was ask Why should Dr. [redacted] wait for me, when he only finances for 3 months for his other patients, these payment agreements were after the work was done. [redacted] his office manager repeately attacked my character, saying "Shame on you for not paying Dr. [redacted] the fee he demands, before the work was done. I was sitting inthe chair, the dental aide gave me the fee papers and said someone from finance would come right away and speak with me, only after everything was done did the finance [redacted] manager speak with me, encouraging me to apply for their health credit card, I signed the papers agreeing to pay 500.00 down and 375.00 monthly. I left, today when I wanted to find out when my bridge would be ready, I was informed that Dr. [redacted] wrote me a letter, explaining that he wanted his money, first. I have not recieved this letter.Desired Settlement: I want Dr. [redacted] to complete my bridge work for the agreed amount with 375.00 dollars a month payment. I want all my dental records and x-rays so that I can go to a different dentist inthe future without repeating all the films and molds. I would like an appology from his Manager [redacted] and I want to warn future patients of this Dr.s' pay for service scam.

RE: 10

To Whom
It May Concern;
Attached please see
the copies of the original letter sent to Mrs. [redacted] along with a copy of her
acknowledgment of our financia
l policy.
Mrs.
[redacted] scheduled her appointment via the telephone; she had previously been given a copy of a
treatment
plan created on 02·14·2012, which at the time said she was planning to have the treatment in
in
June 2012.
A
t the appointment Mrs. [redacted] complaint is from, she also asked to have Zoom Tooth Whitening, a
cos
metic procedure that requires full payment at the time of service ($600.00). This was the situation
which she was
told that the office finance person would speak with her, the conversation did not need to
take place because
she told the assistant she was planning on paying the amount as required at the visit.
Mrs. [redacted] left our office in a very hostile mood. She was not interested in learning of the
Care CreditInterest
free
credit card we offer and was curt making a statement somewhat saying ... "well the work is
done and
this is the arrangement I am able to afford!" As you can see from previous treatment plans
and our Office
Financial policy the patient was aware payment is due at time of service.
Mrs. [redacted]
left our office with a beautiful temporary bridge which she herself said she was delighted
with. Due
to the fees our office incurs we wi ll need VI of the appointment fee paid prior to sending the
case for the porcelain fabrication. We
assured Mrs. [redacted] that in the event the temporary gave her
any co
ncern it would be addressed. This arrangement also extended her the privi lege of continuing
auto deb
its from the HSA as origina lly planned.
Mrs. [redacted] interpretation of the conversation is much different tha n the 3 witnesses here at our office
who overheard word for word the expressions of the Office Manager (Geri). Mrs. Stevens on the other
hand was raising her voice, accusing Dr. [redacted] of just being mean and harmful and taking advantage
of people's money, not interested in helping ..... [redacted] did say shame on you for her accusations of Dr.
Sorensen being an uncaring provider just wanting money.
Our resolution is beyond fair. Mrs. [redacted] is aware of the financial policy, she received services which
she agreed to have done, and the fees incurred are her responsibility. We will gladly send her case for
the permanent Bridge when the $2067.00 is received. We do accept Mastercard, Visa and Discover.
Sincerely,
June

RE: Financial Policy
Dea r [redacted]:
It has been brought to my attention that you obtained dental services by me on 6-
13-2012
without an agreed payment arrangement. It is my understanding that
you were prepared to make bi-weekly payments through an automatic debit
arrangement. Unfortunately, that arrangement far exceeds my ability to do "inhouse"
financing for you. My office staff had suggested Care Credit to you which
you
declined.
Your treatment was for Zoom and a Four- Unit Bridge and the appointment was
for over 2 hours long. When your case gets sent to the lab I will incur the
laboratory expenses that must be paid prior to the case shipping back to me.
You have a Temporary Bridge that will remain in function, until the Permanent
Bridge is cemented.
Once you have paid
1/2 of the balance due ($2067.10), I will send the case to the
lab. We can then get you in to cement the permanent restoration. At this time
you may continue to do the bi-weekly auto withdrawals as arranged for the
balance.
If you would like to make an initial"bulk payment" to expedite the bridge, we do
accept
Mastercard, Visa or Discover. The Care credit is also a more affordable
interest free option which you can apply to at [redacted].
Until the balance is down by
~ it will not be necessary for you to have any
appointments, so the July 9
th appointment is cancelled.
Enclosed for your reference is a copy of the financial arrangements we offer and
your original acknowledgement.
sincerly,
Dr. [redacted]
