The customer was refundedI thought everything was taken care of

I received the refund

She came into our clinic on January 9, and spoke with a manager who explained the reasons for early cancellation She stated she would just cancel her credit card On January 1, she went into the *** location and produced medical documentation but she never filled out the required cancellation form.See the attached documents that were faxed to us from the *** location with the date and time of the fax on the bottom the documentation also has a time line in her handwriting which states when she called and when she actually came in to request the cancellation This timeline also shows that she did not try to cancel her membership prior to her initial membership dues being drafted on 12/15/

We have been trying to get [redacted] to return the cancellation form so that we can attach the doctors note and cancel her membership.  She was aware of what she was signing and explained our policy to her.  We did give her a copy of her membership agreement, because we send home a...

member packet with all this information.  We can not refund her money because that was her monthly payment that she agreed to pay by signing the agreement, and the other charge was from when she received services on her first visit.  Now her account is in default because she has cancelled her card.  I have attached the contract that she signed and initialed.

This lady was my first customer of the day luckily, yes she order the soup and she didn't pick it up ,and I am 100% positive that she gave me a ten dollar bill not a twenty because I counted the cash in my register a few minutes before she...

walked in and it was $150 which I normally leave in the register and I counted the cash after she claimed that she gave me the 20 dollar bill and it was $153.28 which means I gave her the right change, secondly when I counted my cash I had no tens in my register prior to her transaction,one more thing made me sure that things not usual with this old lady ,when she took the money out of her purse to pay she had a punch on one dollar bills and a punch of fives and lots of coins and I was wondering to my self why she is skipping all the ones and the fives and gave me the ten to pay which normally people do ? I believe it's one of two things ,either she is tool old to remember things and she really believed that she gave me a twenty ( I think she is in her mid-eighties)or she has her way of making easy money !regardless we do give free food sometimes for hungry people with no or short on their money and we also give cash for stranded people who asks us, any ways we still like to earn her as customer and help her with food or cash if needs it !

we owe you a cup of soup , you are more than welcome to come and have it and a twenty dollar bill on top of it if that makes you happy, 
I still believe that I didn't take your twenty but willing to close the subject from my own money because I like to make you happy

This membership that was purchased for [redacted] was actually purchased by her boyfriend [redacted] that was the manager at Massage Envy Spa [redacted]. It was his credit on file.   He was the one that activated her account and checked the auto renew membership option button....

 It was [redacted] responsibility to cancel this membership, since he was manager at the time he knows what company policy is.  The money has been returned to his account.Thanks and have a great day.[redacted]Manager

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint. 
Their reply is inaccurate and untruthful. I did not receive a package to take home with me; they kept a copy of my contract themselves. I have a witness who was present with me who can determine this. I called them multiple times to have them cancel my account and was told each time that the manager was not present and would call me the next day. I have only as of yesterday, January 19, received an emailed copy of the cancellation form. As I do not have a printer, I was only able at this date to print out that form to return.
