Review: For several years my mother has been receiving services provided my JEVS. And over these past several years there have been at least twenty caregivers. The following are all things that her case manager/ service coordinator [redacted] was fully aware of but never took action upon. Over the past several years caregivers have been scheduled to work and have either not come into to work and asked me to clock in for them do the job and clock out for them, or they'd come do some work and then leave before noon and have me finish the rest of the job for them. At times when I wasn't around she'd just be left in bed not fed not bathed nor recieved her medication. There was an incident with one of the caregivers [redacted] gossiping to her niece who worked at [redacted] with my cousin about things that were going on in our home. Which resulted in issues for my cousin at her work place. That was in 2008 and one of the first things I informed [redacted] of. I apologize if things will be out of order. Another issue [redacted] was aware of is regarding [redacted] and [redacted]. [redacted] was a hostile employee whom would threaten other employees, myself, my mother, individuals in public while with my mother. [redacted] would come to work intoxicated and impaired due to marijuana. There were times she'd bring bottles of wine and other alcohol and consume them in the living room in front of my mother. As well as using marijuana. Earlier this year when my mother let [redacted] go she torn things down off the bookshelf and got into my mothers face as if to fight her. When she walked out of the apartment she called my mother as well as had her boyfriend call my mother. Her daughter made post on my [redacted] page. I received well over forty blocked calls from her and her friends all hours of the day for well over a week. Ultimately we had to change the locks. Last year while my mother was in the hospital [redacted] would take the apartment key and enter with her boyfriend to clock in and out. [redacted] was a big bully. [redacted] is [redacted]'s sister whom also became a caregiver for my mother. On a daily basis the two would demean my mother for being incontinent. During their time there a lot of items were reported stolen. The spring/summer of 2011 would time that I spent most working the caregivers shifts for them mainly [redacted]. This wasn't a once in a blue moon occurrence this was something would happen on a weekly basis several times a week. I did in fact request not to work my mother would call my several times and beg me to work. And honetly there have been times in the past when we've called the company to have someone work and they would say they didn't have anyone. So much stuff has gone on through the years this is some of the more serious stuff. When calling [redacted] you can guarantee you'll get her voicemail and hear back from her weeks later if at all.Desired Settlement: I would like the money that is owed to me as well as reinstatement to work in healthcare

After receiving the complaint from the RevDex.com, JEVS Director of Supports Coordination ([redacted]) reviewed all

of the case notes from 2008 to present on [redacted].

**. [redacted] is enrolled in the Pennsylvania Attendant Care ACT

150 Program, which is part of the Home and Community Based Services overseen by

the Office of Long Term Living. As a Participant in ACT 150, **. [redacted] has

elected to be the employer of record for her Direct Care Worker’s (DCWs) under

this model (called the consumer model) the Participant (**. [redacted]) is able to

hire, fire and make all management decisions independent of JEVS Supports for

Independence. JEVS was **. [redacted]’s Financial Management Service (FMS) from 2008

– December of 2012. In December of 2012, **. [redacted]’s FMS transitioned from JEVS

Supports for Independence to Public Partnerships LLC.

JEVS Human Services has been and continues to be

**. [redacted]’s chosen agency for Service Coordination since 2008.

Findings include consistent home visits, monitoring

telephone calls and additional contacts where inquiries were made regarding the

Participant’s (*. [redacted]’s) satisfaction with the program and the services she

receives.

While **. [redacted] was with JEVS Supports for Independence

Fiscal Management Service, no complaints were filed by **. [redacted] regarding her

dissatisfaction with the Direct Care Workers that she had chosen to hire. There

is no documentation on file that at any time the Participant voiced her dissatisfaction

with the quality of care she was receiving, nor were there any reports of

theft, drug use, tardiness, neglect, threats, or hostility towards her by her

hired Direct Care Workers.

In January 2013 a report was made by [redacted]

[redacted] to [redacted] (Service Coordinator for *. [redacted]) regarding a

dispute with former Direct Care Worker [redacted] who was accused of making an

excessive number of phone calls to the Participant’s home in retaliation for

being fired.

[redacted] spoke with the Participant regarding the accusations,

who directed [redacted] to speak directly with [redacted] in

regards to the allegations. [redacted]

contacted [redacted] (who denied the allegations as she had already found

another position as a DCW) in an attempt to advocate on the behalf of the

Participant and end the excessive phone calls being placed to the Participant’s

home.

[redacted] instructed the Participant and the Participant’s

daughter ([redacted]) to contact the police if the phone calls

continued and to file a harassment charge.

[redacted] gave the Participant the choice of changing her model

of care from Consumer Model to Agency Model, where the Direct Care Staff would

be sent out and managed by a local agency instead of **. [redacted] handling

everything herself. **. [redacted] declined the proposal to switch models at time

JEVS is not the current fiscal agent for **. [redacted]. The

desired Outcome/Settlement to the Revdex.com complaint is “to be

paid money owed to me as well as reinstatement to work in healthcare” JEVS

Supports for Independence is no longer in business as a Fiscal Management

Service for Participants Enrolled in the ACT 150 Program. JEVS Supports for

Independence has no open record of unpaid timesheets to [redacted]

[redacted] for services rendered to *. [redacted].
