***, purchased a policy from agent *** *** with *** LifeThe *** Life
policy was replacing another policyThe policy being replaced by *** Life was
not canceled by Mr*** which created a situation where both insurance
companies drafted premiums
Mr
*** then contacted *** *** to cancel the *** policy*** ***
informed Mr*** that he would need a written statement from himJL Smith
Associates cannot cancel a policy verbally and must have a written statement
This is mandatory with all insurance carriers as we are not authorized to
conduct this type of transaction on behalf of a policyholder

written statement from Mr*** was not received by our officeOn Nov
5th Mr*** again called the office and spoke with *** ***
Again, *** *** explained to Mr*** that we needed a written statement in
order to request cancellation*** *** also informed him that as soon as it
was received that he would fax it to *** life and follwith them to insure
that the request was received*** *** also informed him that the process
could take a couple of weeks
We
received his written cancellation request on November 7thHis
cancellation request also stated that he wrote a complaint to the Revdex.com
simultaneously The letter was immediately faxed to *** Life the day we
received itPrior to receiving Mr***’s written statement there was nothing
we could do to resolve the issue with *** Life
Mr
***’s request was processed by *** and a refund check was mailed out to him
After receiving the complaint from the Revdex.com *** *** called Mr*** to let
him know that the request had already been sent to *** Life.
Mr
*** then informed *** *** that he had already received a refund check from
*** Life the previous Saturday. During that phone call *** *** addressed the
fact with Mr*** that once he actually sent the required documentation to us
that the refund request was processed just as he had originally explained to
him Mr*** agreed and admitted that he probably jumped the gun filing a
complaint with the Revdex.com
