[redacted] erroneously referred a balance I had paid to a collections agency. Without my knowledge, the agency, [redacted], subsequently reported the amount as unpaid to at least one credit bureau on July 1, 2014. This inaccurate report was brought to my attention by my employer.

[redacted] administrative staff acknowledged that their record keeping has been disorganized and that a mistake had been made. Nevertheless, after more than a month since bringing this matter top their attention, the false report still sits on my credit report and I have not received any follow- up communication.

I was looking for a new dentist that was in my Dental Insurance Network. Having received a recommendation, I contacted Dr. Yeargan's office and was assured that he was in my network, so I scheduled an appointment for Dec. 10, 2012. Upon arriving for the appointment, I presented my insurance card and was again told that Dr. Yeargan was in my network.It turned out that he is not in my network. My insurance paid him 50% of the insurance discounted bill as an out of network provided. I sent Dr. Yeargan a letter explaining the situation and included a check matching the 50% payment made by the insurance. So Dr. Yeargan has been paid 100% as would have been allowed by the insurance.Dr. Yeargan's response was a new bill for an additional amount of $211.00 with a threatening note for me to pay it or be be turned over to a collection agency.
