Our accounting records show that this account holder was sent 6 statements for an outstanding balance after their insurance had cleared (dates: 9/26/13,  10/30/13,  11/26/13,  1/29/14,  2/27/14,  3/26/14).  With no response after 6 statements and no...

undeliverable mail returned to our office, we sent this account to collections for the outstanding balance plus finance charges ($6.41) which had accummulated over these 6 months plus the collection fees that had accrued when we referred this account to the collection agency($50.00). This is standard operating procedure by our office for past due accounts.
   Our financial policy, which this account holder signed and received on 8/29/12, clearly states that if a balance becomes past due and the account is referred to a collection agency that the account holder agrees to pay all of the collection costs that have accrued.  We did, however discover that this account holder had not signed our most updated financial policy at their last appointment which now states that finance charges will start accruing after an account is 30 days past due.  Because the most recent financial policy was not signed by this account holder, we will gladly refund this account holder the finance charges that accrued over the 6 months that the account was past due which totals $6.41.

Company responded by phone. **
The company indicates they mailed the consumer a check for the interest. It has not been cashed.  If not received company will put a stop payment on the check.  In regards to the other requested refund of $50, the company is willing to split the cost and refund the consumer $25. The consumer must come into the office to pick up the check.  The consumer did sign a financial responsibility form, and though it is not the newest one it is still effective.

Since they acknowledged I had "not signed our most updated financial policy", I am seeking the $50.0o0 collection fee and want it taken off my credit report in addition to the $6.41 they already agreed to refund.
As for them mailing me me and nothing coming back undeleiverable, I do not work for the USPS and cannot be held accountable for others actions. I still to this day have not received anything from their office. I only knew of this bill when I receieved a telephone call from the collection agency.  If my account was delinquent why have I never received a telephone call from their office alerting me to the issue. I am sure at some point during this time either the office manager, admin, or billing department had to have at least a few minutes available that they could have could the home and spoke to myself, my husband or left a message on the answering machine. 
Again in closing I am requesting a refund of the $50 collection fee, $6.41 finance changes, and to have this removed from my crredit report.  Legally I cannot be held in contempt or neglegient if I was not aware of the bill in any way other then their office repeatively mailing me for months and haven't signed or been given a copy of their own updated forms.
