I am unable to get a copy of my mammogram report (from 2013) and images sent to SimonMed located at *** * *** ** *** *** *** ** XXXXX They have sent three requests since December of Most recent being 02/10/ I am facing breast cancer and Carondelet is ignoring the request for medical records They also refuse to mail these to my home address in Phoenix I can not take time off from work to drive over miles to get my records
