The patient stated that he received a bridge from us however this is inaccurateI believe he is actually referring to his top and bottom partial dentures because in our system he did have an appointment on and his partials had not returned from the lab at the timeThe patient did however receive his top and bottom partials the very next week (414)He returned two more times for adjustments (and 52014) but left satisfied at his visitWhen he returned on he did not present any concerns regarding his partialsThat was his last visit at our officeRegarding [redacted] 's balance, his ledger is detailed and explains his financial responsibilities for every visitIn addition, he has also received an Explanation of Benefits directly from his insurance company which outlines what he is responsible forWe receive the same documentation from the insurance company that he doesWe are always available to review these documents as well as the ledger with [redacted] in person, if he can find the time to come into the officeTo provide a summary: For date of service - $was his responsibility due to his insurance copay and deductibleThe patient made no paymentsFor - $was due to his insurance copay and deductibleThe patient again made no paymentsFor - $was due to his insurance company stating that the patient was completely responsiblePt made a payment of $For (the time of his top partials) - $was his responsibility due to his insurance copay and deductibleThe patient again made no paymentsFor (the time of his bottom partials) - $was due to his copayPt made a payment of $Pt has not submitted any other payments since thenThe patient had a balance prior to starting work on his partialsHis last payment was almost a month before the visit where [redacted] 's partials had not yet come back from the labI am unable to speak on the person her refers to as having a poor attitude since I was not present for this exchangeWe do apologize and assure you that this behavior is not tolerated at our officeWe would, however, like to resolve [redacted] 's balance immediatelySo if [redacted] is able to remit payment we would greatly appreciate itIf he is not satisfied with the fit of his partials then he is free to come in for more adjustmentsWe do not charge patients for any necessary adjustments to denturesIf there are any further questions, please contact us

At Dental Smiles Unlimited, we try to make our patients' lives convenient by allowing them to schedule an appointment for their month follow up well in advanceThis is because appointments go very quickly and we tend to be booked months outSo by offering the next appointment when the patient is
checking out, it allows them to have first priority on the date and timeWe do not schedule appointments without the patient's knowledgePatients must provide the date and time for us to schedule into the systemOnce an appointment is entered, patient's receive email, text and phone call reminders to let them know of their upcoming appointment and to contact us to confirm, reschedule or cancelPatients have up to hours to call and cancelThis gives us sufficient time to contact and appoint another patient for that time slotIf a patient does not call to cancel their appointment or they contact us in less than hours prior to the appointment, they are charged a broken appointment feeWe would rather a patient that is available to make the appointment instead of an empty chairThis info is also included in our Financial Agreement form which is given to and signed by all new patientsEven considering all of this, we still provide the courtesy of waiving the first broken appointment feeWe do this for every patient the first time they miss an appointment*** *** fee was waivedThe ledger reflects no balance
