Good Morning [redacted] Please note that this is a Doctors Facility and we are extremely busy so this is our finalnotice regarding this issue in hopes you close this frivolous caseIt is clear EVERYONE has their version of this situation and it was brought to our intention by [redacted] he has a friend who is employed at [redacted] which is why he received such aminformal notice from [redacted] The BOTTOM LINE is that it is our office policy if a patient has a specific lab he should advisethe front desk as we do not have a account with quest so we could not send any specimens thereas said in the last email we give the patient a prescription [redacted] LEFT a urinespecimen which was sent to [redacted] that PARTICIPATED at the time and dueto his HIGH DEDUCTIBLE payment was applied to the deductibleSECONDLY and LASTLY [redacted] WAITED A YEAR TO CONTACT US in that time MethodistHospital tried numerous times to contact [redacted] as said in the last email feel free to confirm thatwith the information provided with the lab they had the same contact information he provided and we have inour systemAGAIN WE REQUEST THIS CASE BE CLOSEDClear [redacted] has a lot of time on his hands to entertain this we do notWe have helped [redacted] ABOVE AND BEYOND WHEN THE BILL HAS NOTHING TO DO WITH USPlease feel free to contact our office manager Lily L***

[redacted] This is the response to ID# [redacted] Customer [redacted] [redacted] was a patient of DrL [redacted] back in March According to hiscomplaint FIRST AND MOST IMPORTANTLY Please address this immediately the withthe accusation of the office being fitly DrL*** rented office space in this office before [redacted] paid he had the opportunity to leave if the office was "filthy" and if so whywould you return for a second visit to a filthy office? This office was a MULTI SPECIALITY OFFICE withnumerous doctors and FYI the office is still open @ [redacted] *** DrL***has moved into his new office of his own that is why he longer is thereSECOND stating our staff was rude and unprofessional we helped [redacted] above and beyond even when he called our office being extremely rude and becoming very harassing and was upset when we told him we would report his harassing.Our officeCollects over urine specimens a day if the patient request a particular lab WE DO NOTCOLLECT A URINE and normally we give the patient a prescription so they can go to the lab of therechoiceThis was a YEAR AGO so if [redacted] left a urine specimen we would have sentit to [redacted] which at the time they were participating with his insurance [redacted] We contact [redacted] in behalf of [redacted] and spoke to Magalia @ 718/780-3645who stated the bill was due to the HIGH DEDUCTIBLE [redacted] hasPlease feel free to contact [redacted] She also stated they tried numerous times to contact [redacted] The prices for [redacted] for a Urine Culture is and the Urinalysis is TOTALING [redacted] is stating he only had to pay if we sent it to Quest which is FALSEWe contacted Quest @ [redacted] so you may confirm with Quest [redacted] would have been charged for the Urine Culture and theUrinalysis TOTALING a differenceWe Then Called [redacted] Insurance [redacted] ins@ [redacted] maybe he can provide you or way you with his ID and you can speak with his insuranceand confirm his HIGH DEDUCTIBLE which [redacted] told us WHERE EVER WE SENT THE URINE HE WASGOING TO HAVE A BILL So with that being said our office WENT ABOVE AND BEYOND to help [redacted] with this issue that had nothing to do with usLastly maybe if [redacted] would have answered to the NUMEOUS times [redacted] called and sent billsto him maybe it would have not been sent to collection affecting his credit as he statesYou may call [redacted] and confirm they have the SAME address [redacted] provided in his statement to the Revdex.comSo we ask that you please CLOSE this COMPLAINT IMMEDIATELY WHICH IS AND UNFAIR TO DRL [redacted] AND HIS STAFFShould you need any more information please contact our office anytime and speak to lily the office manager @ [redacted]

Good morning [redacted],Please see below for my response as well as the attached supporting documents.I returned to see Dr. L[redacted] because to start seeing another doctor would have taken more time and cost more money. I called Dr. L[redacted]’s office numerous times to follow up because for 2 months I was told that I needed to speak with Lily who handles billing and not once did she return my call. The receptionist was rude, talked over me, would not provide information, would not take note of information that I was trying to provide, hung up on me and threatened to report me for harassment. Follow up phone calls are not harassment, it's common professional procedure. Not returning a clients phone calls for 2 months on the other hand is not professional. After I was told that following up with phone calls was harassment I contacted Revdex.com and did not call the office. I have no intention to "harass" anyone, I just want this resolved.The information provided by Dr. L[redacted]'s office in regards to urine sample is incorrect. I have attached a letter from my health insurance company [redacted] Health verifying that my co-pay for Quest Labs in 2016 was $20.I am also attaching a screenshot of a message from my health insurance company [redacted] Health that states they have no record of the provider calling about my claim. The address that both [redacted] and the collections agency MBI had was missing my apartment number until I called them and updated it while researching this issue.The reason this issue exists is because Dr. L[redacted]'s office failed to handle the urine sample properly in the beginning. Based on the above facts which have been verified by my health insurance company [redacted] Health I maintain that Dr. L[redacted] should be responsible for payment to settle this bill.Thank you,Ben

We Then Called [redacted] Insurance [redacted] ins@ [redacted] maybe he can provide you or 3 way you with his ID and you can speak with his insuranceand confirm his HIGH DEDUCTIBLE which [redacted] told us WHERE EVER WE SENT THE URINE HE WASGOING TO HAVE A BILL.  So with that being said our office WENT ABOVE AND BEYOND to help [redacted]with this issue that had nothing to do with us. Lastly maybe if [redacted] would have answered to the NUMEOUS times [redacted] called and sent billsto him maybe it would have not been sent to collection affecting his credit as he states. 1. I do not know anyone (friend or family) who works for [redacted] Health Insurance, nor have I ever. This assertion is either an error on Dr. L[redacted]'s part or a lie. 2. As previously stated, I requested that my lab be sent to Quest when I checked in at Dr. L[redacted]'s office and presented my insurance card. The fact that they did not follow my initial request is the cause of this issue. If they did not have an account with Quest Labs and their protocol was to give a prescription than they should have advised me at the time of my visit.3. As previously stated, I only became aware of the bill when a debt collector (MBI) reached out to me at the beginning of April 2017 at which point I immediately contacted Dr. L[redacted]'s office, [redacted] Health and [redacted]. The woman who I spoke to at [redacted]'s lab did not have my apartment number on file which she assumed was the reason that I had not received the bills earlier and that it had gone to collections.4. It's important to note: Dr. L[redacted]'s office originally claimed that I did not have a $20 co-payment from Quest Labs in 2016. I verified this with a letter from [redacted] Health that Dr.  L[redacted]'s office claimed to be "forged" which it was not. I then had a representative from [redacted] Health call Lily L[redacted] to verify my 2016 $20 lab test co-pay. After this Dr. L[redacted]'s office acknowledged that the information that I had been trying to provide to them since the beginning of April 2017 was in fact correct.5. Upon realizing that I was correct Dr. L[redacted]'s office has changed the facts that they are disputing. Originally they claimed that I did not have a $20 co-pay. Now that they acknowledge that I did in fact have the $20 co-pay they claim that I did not request that my lab be sent to Quest, which I absolutely did. Dr. L[redacted]'s office made an error and they need to take accountability for it.6. This is a tremendous waste of my time as well. With that being said, the fact that looking for accountability for Dr. L[redacted]'s egregious business practices is time consuming is not an excuse for Dr. L[redacted] to get away with this sort of business practice and this is why I have contacted the Revdex.com. Patients deserve to be treated fairly and respectfully and Dr. L[redacted] must be accountable for the conduct of his office. Please do not hesitate to contact me if you have any questions or if I can clarify anything further.
