Due to a very high amount of credit card and insurance fraud, we have been advised to scan driver's licenses and insurance cards into our system. We have had people come in and claim to be other people and try to use their insurance. With a picture ID on file, we are able to
discover that the person seeking treatment is not the person's whose ID we have on file. All of this was explained by *** *** and myself to Mrs***. Her husband was not in an emergency medical situation but was seeking treatment. I explained to her several times that we did not refuse treatment. She chose for her husband not to be seen because she did not want to follow the protocols we have in place. We are a private business and if patients want to seek treatment with us they must follow the protocols we have in place to run our business properly. I have since contacted other medical facilities all of which have told me they require that an ID be scanned into their systems

I guess we are forced to buy him a second gift card since he denies getting the first one we mailed out

I am rejecting this response because: I never received the gift card. I need the $25.00 either in the form of a card or a check. I'm willing to go pick it up in order to avoid this problem again. I do not know if the mail delivered to another wrong address or what happened but I haven't received my gift card.

mailed his gift card on 5/5/15.   She also spoke to him when he did come back in and

verified his address and we did not get any returned mail.  What I suspect happened is another family member opened the mail and got the gift card.  We have kept a log and he was mailed a gift card to the address on file that he says is his address on 5/5/15.
