This is the third compliant Carver Bank has been in receipt of, regarding this
matter on behalf of *** ***As stated in our response submitted on June
*, 2015, Carver Bank has investigated this matter and apprised *** ** ***,
who submitted the Affidavit of Forgery, that our investigation revealed our
customer New Horizon provided information that services were rendered to *** *** for repairs to a vehicle
In the third letter received, *** *** is stating the check in question was
drawn on Capital One BankThe Affidavit of Forgery received by Carver Bank and
signed by *** *** (dated August **,2014) clearly denotes the check is drawn
on *** ** ***Carver has a copy of the check in question, and it is duly
noted the check was issued by *** ** ***, Government Employees Insurance
Co is the maker of the account at *** ** ***The check duly notes this
check was issued for collision coverage, claim number:***
Carver Federal Savings Bank has reviewed this matter with our Legal Division
and our decision to not honor the forged endorsement claim still standsWe
have been in communication with *** ** *** accordingly

Please note the attached response that I sent to Carver Federal Savings Bank They have not taken care of my request in a satisfactory manner I acknowledge receipt of Ms S***'s letter dated May **, received on June ** Again the questions asked were not answered and MsS***'s letter dated May 20, added to that listI never received a ira contribution form from 2012- Why?Why was the form sent now after May, since tax returns for were completed prior to April If the attached statement represents amount in account to December, and adding the first two months of 2016, again it is not correct The IRS will think that I did not reinvest the funds in time from December, to March, If I get audited or penalized by the IRS,I will direct them to Carver I am again requesting a form for all the prior years and the form indicating only numbers and a correct form for for my taxes of this year in order that the IRS knows when the funds were taken out since I have the documentation that the funds were deposited the same day taken out.I do not wish to be penalized or audited by the IRS because of your errors if you do not understand what I need to correct this please contact someone who can explain it
