On 10/11/2016 I went to visit this Dentist for a deep cleaning and fillings, there they told me that I would be a perfect candidate for Botox. The Botox would relax the muscles, and help stop the grinding of my teeth. They explained that they would take care of everything, and never mentioned that...

my insurance would not cover the procedure. I was told that it was only going to pay $450.00 out of pocket. On 10/24/2016 I had already received the Botox twice, and to my surprise the total bill was $2311.00 with $129.00 a month. This was not what they had explained to me two weeks prior, and after speaking with [redacted] she made it clear that Botox is not covered by insurance. Which means that they knew it was not covered, and never fully explained that to me. I have already had 2 procedures, and are willing to pay the $450 which is what I was told it would cost, not $2311.00.

We go out of our way to help our patients. Though it is unfortunate that we have this complaint against us but please find attached documents that clearly shows that patient knew that he was responsible for the rendered treatment.We are very diligent with financials and make sure our patients not just review the documents but actually “sign” them.We even make them “initial” on the statement that states that “Insurance benefits are an estimate and an insurance may or may not cover a procedure. And any portion not covered by insurance becomes patients responsibility”. Documents attached are: -          Office Financial Policy: We reviewed the general financial policy and the patient signed it on 10/8/2016. This policy states that “ultimately patient is responsible for all charges regardless of any dental coverage”. -          Signed Treatment Plan: Before rendering any treatment, we present the treatment plan to the patient and go over the financials and insurance coverage. As you can see in this attached treatment plan which clearly shows that the Insurance does not cover BOTOX as a procedure. The insurance covered other procedures but not BOTOX. Since this document was signed by the patient, it shows that patient knew that it was not a covered benefit and he needed to pay out of pocket. The patient signed the treatment plan with that knowledge. In fact the patient paid all his portion on the date of service. We do not collect any fees that we know are covered by insurance. On behalf of the patient we bill the insurance for that payment. In this case it was pretty clear that the dental insurance does not cover cosmetic procedures and the patient paid accordingly. Not sure why this concern is coming up now. With these evidence documents we believe all the transactions were done appropriately and the patient reviewed the fees and paid the fees appropriately. Please let us know if there is anything else needed from our end. Thanks again for helping out in this complaint! Regards,[redacted]
