I am in receipt of your letter dated April 22, 2014.
I have only met Mr....

[redacted] on one occasion. The pet dog he refers to ls "[redacted]" which, in our records, the owner is noted to be Ms. [redacted]. "[redacted]'had a severe, debilitating skin condition that was obviously not primarily allergic. I discussed, at length, types of testing that would help define the problem. An estimate was prepared, agreed to, and signed by the owner. Following the Initial screening. two (2) additional tests were ordered, again with the understanding and authorization from the owner, to evaluate a possible hyperthyroid problem and/or hormonal imbalance. To reiterate, this was discussed with the owner who agreed and presented the pot for these additional testa. The results did not Indicate hyperthyroidism but did confirm a hormonal imbalance.
Having uncovered the pet's problem I communicated this information with Mr. [redacted]'s wife who has always been complementary and expressed gratitude for improving their pet's condition. I also discussed this case with a veterinary dermatologist on the mainland, at my own cost, and planned a long term approach that would proived relief and quality of life for the pet. This was again discussed with his wife who seemed excited that a solution had been found but then never followed up with me or returned further phone calls.
Relative to Mr. [redacted]'s complaint of not being able to have medications refilled, I cannot prescribe long term medication legally or ethically for allergies knowing that this was not the primary problem and by doing so would initiate additional problems.
As to the medical fees, the initial visit was $688.71 within the originally estimated amount. The second bill was for the two (2) additional laboratory fees which was for $298.17. Mr. [redacted]'s total expenditure to diagnose and begin treatment of the root of his pet's problem was $986.88 of which only $62.00 was the consulting/examination fee. The balance was in laboratory, diagnostic and prescription charges. (Copies of signed estimate and billing receipts can be provided if necessary.)
Furthermore, Mr. [redacted] Is filing for benefits from his pet medical insurance companies so there should he reimbursements to him for expenses incurred.
