11/22/and a follow up visit on 12/1/ She presented with her United Health Care (UHC) card, which was entered A claim was sent out by our billing company on 12/5/2013, and again on 3/5/2014. The claim was not adjudicated nor was noted as received On 08/14/there was a note made by a biller thatthe Insurance ID had not been entered correctly, hence the
