Good afternoonIn response to complaint ID number [redacted] this patient presented with no insurance provided on date of service 05/24/The services provided to this patient included critical care that was billed out as a separate encounterThe initial statement was sent to the patient on 06/7/ as a request for insurance informationThe patient called in non-par insurance (Empire) The patient received two additional statements before payment was made and the account was automatically forwarded to collections on 09/20/The patient balance was accurate at the time the account was transferred to collections and the appropriate protocol was followed Since this complaint the patient has filed the appropriate “surprise bill” forms to her insurance carrier and will receive a check for the full patient responsibility amount of $781.32, dated 12/13/Tina has contacted the collection agency (Tice) and has requested they discontinue their collection effortsShe will call the patient and explain what actions has been taken If you have any questions or would like additional information, please let me know Kirstan M [redacted] Practice Manager Maple Road Williamsville, New York Phone: Fax:
