Your cleaning was covered at $272.93 leaving a balance of $21.07. The exam, X-rays and prophy were not covered because it was used by you at another office and therefore was over frequency, which unfortunately you did not disclose...
to us. We have already send you the explanation of benefits received from your insurance company for that day.
So the balance on your account is $192.07 To resolve the situation we will write-off $21.07, the balance from your cleaning as was promised by the hygienist. The new balance is $171.00

This letter was sent to him explaining the reason he incurred a balance of $192.07 despite his insurance paying for the hygiene services. If the patient does not disclose what his insurance has already paid for services elsewhere, unfortunately we are not liable.

I told to the dental hygienist that I had work done in the summer, where he followed up by saying I could come in 3-4 times a year. I told them this but they're denying it, saying I'm confused about my insurance. I understand the charges on my account but when tell someone you are covered for today, I don't expect to pay anything. I wasn't told you might be covered today. I don't understand how you are legally allowed to change you're mind after the treatment. This is what bothers me, not the 192$
