text-align: left;" valign="top"> November 19, 2015 we received an e-mail from Mr. D.C. [redacted]. November 19, 2015 Customer Service Rep. replied to customer’s e-mail requesting a model number so that a correct replacement can be sent out. November 24, 2015 Mr. [redacted] sent back an e-mail providing us with a sku number. November 25, 2015 a replacement was sent via postal service to the P.O. Box provided by the customer. We did not send out certified and assumed that the issue was resolved since we did not hear from the customer. Upon receipt of your mailed form to Chris C[redacted] on January 4, 2016 (due to the New Year holiday) we reached out to the customer with a phone call. A second replacement has been sent to Mr. [redacted]. We requested the street address instead of the P.O. Box so that the shipment can be tracked.
