received an email from himIn November he told us he would resend info for me to sign and mail to him I never received this paperwork In December we contacted office and spoke with a co- worker, who stated Mr*** was still completing my returnIn JanI received returnI signed and mailed to him on 1-6-On Feb 3rd we contacted him to check on status and he stated my paperwork was missing a signature He never contacted me to let me knowOn Jan 5th I went to his office and signed return in personHe was not there when I arrived and had not signed necessary info His Assistant assured me she would mail return
