This form was signed by [redacted] on 04/15/Again, it is an entire form, not something that is “slipped” into other paperwork as stated by [redacted] .There was no purchase made by [redacted] He has not paid anything out of pocket, So there is no refund due[redacted] has only been to this office once for treatmentThe rest of his family has been patients for yearsAll are aware of office policy and nothing has changed during that year periodIf you have any questions regarding this matter, please contact our office at [redacted] .Thank you for your time, [redacted]

I disagree with their answersIn years of going to the dentist, I have never had this treatment until I went to this officeAdditionally, if this was a standard procedure, most dentists would perform it, and insurance companies would not have a coverage limit on itLimits were in place because it is a specialized treatment.If they have not had problems with coverage in the past, they wouldn't have put a specific form in their packet related to this procedureObviously, they have had problems collecting for this in the past, so added a new form to use to collect from patientsIf they didn't have any problems with this before, they wouldn't have needed a new formThey seriously cannot expect your average patient to understand the names and treatment codes of each procedure, and the coverage frequency under the insurance policyWe are not the expertsWe count on them to look after our interest, not use us to fund equipment purchases.Yes, I have only been going to this dentist for a year, but we aren't talking about my treatmentThis is about the treatment for my son who has been going there for years.I have made a payment of $via check, of which they have cashedI can provide proof if needed
